内镜下射频消融联合光动力学疗法治疗无法手术切除的梗阻性肺癌——附34例报告

【摘要】 目的评价内镜下射频消融联合光动力学疗法治疗无法手术切除的梗阻性肺癌的疗效。方法60例腔内梗阻性肺癌患者中34例先行内镜下射频烧灼瘤块,1周后再行光动力照射。另10例行单纯光动力治疗,16例行单纯射频治疗作为对照。根据治疗后呼吸困难和肺不张缓解情况和治疗前后卡氏全身状况评分对比及支气管镜复查肿瘤缩小情况进行评价。结果光动力联合射频治疗组34例患者症状均缓解;治疗后卡氏全身状况评分较治疗前显著增加;CR5例,SR26例,MR3例;总有效率为91%,明显高于单纯光动力治疗组和单纯射频治疗组。无一例出现与治疗相关的死亡。2例出现皮肤光过敏,2例出现发热,2例出现痰中带血。结论射频消融联合光动力学疗法治疗无法手术切除的腔内梗阻性肺癌能有效缓解症状,缩小肿瘤,解除梗阻,改善生活质量。

【Abstract】 Objective To evaluate the clinical value of photodynamic therapy and radiofrequency therapy under endoscopy to treating inoperable obstructive lung cancer. Methods Thirty-four patients with obstructive lung cancer were treated by using radiofrequency therapy and then photodynamic therapy after one week. The changes in dyspnea,atelectasis,Karnofsky performance status scale(KPS) and tumor size under the endoscopy were investigated after the treatment. The patients in control groups received only photodynamic therapy or radiofrequency therapy separately. Results All patients had symptomatic relief. The KPS after treatment increased significantly compared with that of before treatment.Five had a complete remission. Twenty six had a significant remission. Three had a minor remission.And the total efficacy was 91%, which was higher than that of the group of PDT or radiofrequency.No procedure-related deaths were seen.Two had photosensitivity action. Two had fever. Two had bloody sputum. Conclusions Combined endoscopic photodynamic therapy with radiofrequency therapy may be an effective palliative treatment~ for patients with inoperable obstructive lung cancer, which has effect on symptomatic relief,obstruction relief and quality of life improvement.
