基质金属蛋白酶及其组织抑制剂与滋养细胞侵袭性生长关系的研究

【摘要】 目的了解基质金属蛋白酶(MMPs)和组织抑制剂(TIMPs)在滋养细胞中的定位及其在不同滋养细胞病变中的表达和相互调节作用。方法采用免疫组化方法检测MMP-2、MMP-9和TMP-1、TIMP-2在绒毛膜癌、侵袭性水泡状胎块、水泡状胎块、胎盘植入和超常反应胎盘部位等病变以及胎盘着床部位中的表达。结果MMP-2、9和TIMP-1、2主要在中间滋养细胞和合体滋养细胞表达。滋养细胞在胎盘着床部位仅表达MMP-2;超常反应胎盘部位和胎盘植入不但表达MMP-2,而且多数病例MMP-9(+),但阳性强度弱,TIMP-1和TIMP-2(-);水泡状胎块较强表达MMP-2,伴持续性滋养细胞疾病MMP-9和TIMP-1、2(+);侵袭性水泡状胎块和绒毛膜癌MMP-2和MMP-9表达明显增强,TIMP-1和TIMP-2多数(+)。结论在病理性妊娠中MMP-9活性增强可导致滋养细胞发生过度浸润。MMP-9的过度表达和TIMP-1、2的轻微增加,可能共同增强了肿瘤细胞的侵袭能力。

【Abstract】 Objective To investigate the localization of matrix metalloproteinases(MMPs) and tissue inhibitor of metalloproteinases(TIMPs) in trophoblasts and the interaction in different trophoblastic lesions. Methods The expression of MMP-2,MMP-9,TIMP-1 and TIMP-2 was detected by immunhistochemistry in different trophoblastic lesions. Results MMPs and TIMPs were mainly localized in the intermediate trophoblast and the syncytiotrophoblast.Only MMP-2 was positive in the trophoblast of placental site.MMP-9 was expressed positive but TIMP-1 and TIMP-2 negative in most cases of the exaggerated placental site and the placenta increta besides MMP-2 positive expression.In hydatidiform mole,the expression of MMP-2 was stronger.MMP-9 and TIMP-1,2 were positive in the cases with the persistent trophoblastic disease.There was obviously increased expression of MMP-2 and MMP-9 in invasive hydatidiform mole and choriocarcinoma but TIMP-1 and TIMP-2 were mostly positive.Conclusions The increased activity of MMP-9 in pathologic pregnancy is responsible for the excessive infiltration of trophoblast.The excessive expression of MMP-9 can improve the invasiveness of the tumourous cells together with the light increment of TIMP-1 and TIMP-2.
