【摘要】 目的:探讨强直性脊柱炎(ankylosing spondylitis,AS)和类风湿关节炎(rheumatoid arthritis,RA)患者外周血T细胞亚群的变化特点及临床意义。方法:收集AS和RA及健康志愿者外周抗凝静脉血,以ECD标记的抗CD3单克隆抗体和PE-Cy5标记的CD4、CD8单克隆抗体作双色流式细胞术,分析AS和RA患者CD3+CD4+、CD3+CD8+T细胞百分率。结果:AS患者CD4+T百分率和CD4+T/CD8+T比值均明显低于正常对照(P<0.05),CD8+T细胞百分率明显高于正常对照(P<0.05);RA患者CD4+T和CD8+T细胞百分率明显低于正常对照(P<0.05)、CD4+T/CD8+T比值明显高于正常对照(P<0.05)。AS患者的CD8+T百分率明显高于RA患者(P<0.05),CD4+T/CD8+T细胞比值明显低于RA患者(P<0.05)。结论:AS患者外周血表现免疫功能低下,以及RA患者外周血呈现免疫功能亢进,可能都与其免疫发病机制有关;CD4+T/CD8+T比值的变化特点有助于AS和RA的临床鉴别诊断。更多还原

【Abstract】 Objective:To study the variation feature and clinical significance of T cell subsets of peripheral blood in AS and RA patients.Methods: The ratios of CD₄⁺/CD₈⁺ in peripheral blood,percentages of CD₄⁺and CD₈⁺ T cells were analyzed by immunofluorescence staining and bicolor flow cytometry（FCM） in vitro.Results: The ratio of CD₄⁺T/CD₈⁺T cells in AS cases was obviously lower than healthy control（1.73±0.33）.FCM showed that the average percentages of CD₄⁺ and CD₈⁺ in AS was 32.72%±3.72% and 27.40%±5.03% versus 40.15%±4.63% and 23.73%±4.10% in healthy control.The average percentage of CD₄⁺ decreased while CD₈⁺ increased obviously（P<0.05）.The average percentages of CD₄⁺（33.53%±3.70%）and CD₈⁺（15.48%±2.38%） in RA versus healthy control were decreased significantly,while the ratios of CD₄⁺T/CD₈⁺T（2.43±0.13） increased obviously（P<0.05）.The average percentage of CD₈⁺ in AS cases was much higher than that in RA cases（ P<0.05）,the ratios of CD₄⁺T/ CD₈⁺T in AS patients was very lower than that in RA cases（P<0.05）.Conclusion:The findings suggest that immune hypofunction in AS patients may be relevant to its immunopathogenesis,and Immune hyperfunction in RA may also be concerned with its immunopathogenic difference.Therefore the feature of the ratios of CD₄⁺T/CD₈⁺T can be helpful for clinical differential diagnosis as well as cases observation.更多还原
