输卵管妊娠时血清β-HCG水平与滋养细胞侵入输卵管壁深度关系的研究

【摘要】 目的:探讨输卵管妊娠时血清绒毛膜促性腺激素β亚单位(-βHCG)水平及停经时间、附件包块大小与滋养细胞侵入输卵管壁深度的关系。方法:行输卵管切除术的60例输卵管妊娠患者,用化学发光免疫分析法于手术日测定血清-βHCG浓度,手术切除的输卵管行病理检查其滋养细胞侵入输卵管壁的深度,以观察两者间的关系。结果:①Ⅰ、Ⅱ、Ⅲ期滋养细胞侵入分别为24例(40%)1、8例(30%)、18例(30%),-βHCG水平分别为707.2±196.0 U/L、2529.2±563.3 U/L1、0380.2±4598.2 U/L,3期之间-βHCG比较差异有显著性(P<0.05)。②随着-βHCG水平的升高,滋养细胞侵入输卵管壁的深度加深,-βHCG水平与滋养细胞侵入深度呈正相关(r=0.704,P<0.01)。③停经时间和包块直径不同的各组患者血清-βHCG水平及肌层侵入深度比较,差异均无显著性(P>0.05)。结论:①血清-βHCG水平的高低与滋养细胞侵入输卵管壁的深度呈正相关。②滋养细胞侵入输卵管壁的深度与停经时间、附件包块的大小无关。

【Abstract】 Objective:To explore the relationship between serum β-HCG level and the depth of trophoblastic infiltration in tubal pregnancy.Methods:60 patients received unilateral salpingectomy for tubal pregnancy.Their serum β-HCG levels were determined on the day of surgery by using the electrochemiluminescence immunoassay.The fallopian tubal specimens were assessed to determine the depth of trophoblastic infiltration into tubal wall.The relationship was observed between β-HCG level and the the depth of trophoblastic infiltration.Results: ① In stageⅠ,Ⅱand Ⅲ,trophoblastic infiltration and β-HCG level were 24 cases(40%),707.2±196.0U/L,18 cases(30%),2529.2±563.3 U/L and 18 cases(30%),10380.2±4598.2 U/L,respectively.Serum β-HCG levels were significant different among the three groups(P<0.05).②The serum β-HCG levels correlate positively with the depth of trophoblastic infiltration into fallopian tubal wall(r=0.704,P<0.01).③ The depth of trophoblastic infiltration and serum β-HCG level were no different in the gestational age and ultrasonographic diameter(P>0.05).Conclusions:① The serum β-HCG level correlate positively with the depth of trophoblastic infiltration into the fallopian tube.② Gestational age and tubal mass ultrasonographic diameter did not correlate with the depth of trophoblastic infiltration.
