阿尔茨海默病与脑血管性痴呆的诱发电位比较

【摘要】 目的探讨阿尔茨海默病(AD)和脑血管性痴呆(VD)在诱发电位中的特点。方法收集25例AD组、24例VD组及22名正常老年人(NC),应用美国N icolet Sp irit脑诱发电位仪以及声、光刺激,完成听觉诱发电位(AEP)和视觉诱发电位(VEP)检查。结果AD组和VD组及NC组在潜伏期AEP/N1、P2,VEP/P1,波幅AEP/P2、P3,VEP/P2、P3上差异有统计学意义(P<0.05或P<0.01)。与NC组相比,AD组潜伏期AEP/N1、P2,VEP/P1延迟,波幅AEP/P2、P3,VEP/P2、P3降低。与NC组相比,VD组潜伏期VEP/P1延迟,波幅P2和P3降低。AD组与VD组相比,在潜伏期AEP/N1、P2上两者差异有统计学意义,AD组延迟于VD组。AD组VEP的P1潜伏期,VD组AEP的P3波幅及VEP的N1潜伏期改变上与其MMSE评分有关联。结论AD组和VD组的诱发电位变化有类似的异常,但AD组病损范围广,经不同感觉系统诱发的脑诱发电位均有变异。两组痴呆中的3个指标与MMSE关联的特点,值得进一步随访。

【Abstract】 Objective To investigate the differences of visual evoked potentials(VEP) and auditory evoked potentials(AEP) between the patients with Alzheimer′s disease and vascular dementia. Methods Both VEP and AEP were elicited from 25 patients with Alzheimer′s disease,24 vascular dementia and 22 normally age-matched subjects by presenting their visual and auditory stimuli,using a Dantec Concerto SEEG-16 instrument.Results Compared with the AEP and VEP of normal controls,AD group and VD group showed some longer latency or/and shorter amplitued indicators.The AD patients presented significantly delayed latency P1 of VEP and N1,P2 of AEP;The P2,P3 amplitudes of VEP and AEP of the AD were decreased.The VD patients presented significantly delayed latency P1 of VEP presented significantly reduced amplitude P2,P3 of VEP as compared to normal controls.All these variations of BEP had statistical significance(P<0.05).AD patients presented more abnormal BEP than VD as compared to normal controls.The P1 latency of VEP of AD,the N1 latency of VEP and P3 amplitude of AEP of VD were found significantly correlated with MMSE score.Conclusion It was suggested that the variations of VEP and AEP be the biological markes of Alzheimer′s disease and vascular dementia.
