【摘要】 目的探讨胸膜肺切除术在肺结核外科治疗中的应用。方法回顾分析17例重症难治性肺结核患者实施胸膜肺切除术的治疗经过及转归。结果全组一期临床治愈14例(82.4%)。术后胸腔大面积渗血死亡1例(5.9%),支气管胸膜瘘2例,分别经二期胸廓成形术及长期胸腔引流治愈。16例最终获得治愈(94.1%)。全组随访1~6 a,1例术后4 a死于对侧气胸,15例恢复正常生活。结论胸膜肺切除术是治疗某些难治性肺结核病的有效手段之一,但应严格掌握手术指征,术中术后妥善管理以减少并发症,提高疗效。更多还原

【Abstract】 Objective To discuss the application of pleuropneumonectomy in the surgical treatment of pulmonary tuberculosis.Methods The therapeutic procedure and the consequences of pleuropneumonectomy in seventeen patients with serious complicated pulmonary tuberculosis were reviewed and analyzed.Results For the whole group,fourteen patients were all cured.One patient died from diffuse hemorrhage of thoracic cavity.Bronchopleural fistula occurred in two patients,who recovered ultimately through surgical therapy.The total cure rate was 94.1%.Conclusion Pleuropneumonectomy is one of the effective methods of treating some complicated pulmonary tuberculosis.Handling operative indication strictly with careful management can decrease the incidence of postoperative complications,and improve the cure rate.更多还原
