控制性超排卵中血清E2、LH水平对体外受精—胚胎移植的影响
THE EFFECTS OF SERUM E2 AND LH LEVEL DURING CONTROLLED OVARIAN HYPERSTIMULATION ON INVITRO FERTILIZATION-EMBRYO TRANSFER
【摘要】 目的:探讨体外受精—胚胎移植(IVF-ET)周期注射人绒毛膜促性腺激素(HCG)日血清激素水平对助孕效果的影响。方法:回顾性分析110个IVF-ET周期,采用磁酶免技术,测定血清雌二醇(E2)、黄体生成素(LH)水平,分析不同E2、LH水平对用药天数、用药量、获卵数、受精率、卵裂卵、胚胎植入率和临床妊娠率的影响。结果:HCG日不同E2水平组间胚胎植入率和临床妊娠率差异无统计学意义(P>0·05),随着E2增高获卵数增多,各组间差异有统计学意义(P<0·01);E2<1500ng/L组使用促性腺激素(Gn)药量增多,与E2>3000ng/L组比差异有统计学意义(P<0·01)。LH水平<1·2IU/L和≥1·2IU/L组间胚胎植入率分别为26·27%和10·37%,临床妊娠率分别为44·23%和20·69%,差异均有统计学意义(P<0·01);LH<1·2IU/L组用药时间延长,药量增多。结论:注射HCG日血清E2水平与获卵数和使用Gn药量有关,但不能预测IVF-ET的妊娠结局,低LH组有着较高的胚胎植入率和临床妊娠率,但使用Gn时间延长、药量增多。
【Abstract】 Objective: To investigate the effects of serum Estrodiol (E_ 2 ) and luteinizing hormone (LH) level on in vitro fertilization-embryo transfer (IVF-ET) on the day of HCG injection.Methods:110 cycles of IVF cases were retrospectively analyzed. In these cases, the serum E_ 2 and LH levels were detected by magnetic average phase enzyme immunoassay system to study the effects of different serum E_ 2 and LH levels on the days and the dose of gonadotropin, occytes retrieved, fertilization rate, cleavage rate, embryo implantation rate and clinical pregnancy rate. Result:The embryo implantation rate and clinical pregnancy rate were not statistically different among different serum E_ 2 level groups on the day of HCG injection (P>0.05),but the occytes retrieved more when the serum E_ 2 level increased and there were statistically differences among different serum E_ 2 level groups on the day of HCG injection (P<0.01).The dose of gonadotropin was increased in serum E_ 2 <1 500 ng/L group, and was statistically different compared with the serum E_ 2 >3 000 ng/L group. The embryo implantation rates in serum LH<1.2 IU/L group and LH≥1.2 IU/L group were 26.27% and 10.37% respectively, and the clinical pregnancy rates were 44.23% and 20.69% respectively. There were statistically differences between these two groups (P<0.01).The days and the doses of gonadotropin administration were increased in serum LH<1.2 IU/L group. Conclusion:The occytes retrieved and the doses of gonadotropin administration are related to the serum E_ 2 level,but the serum E_ 2 level can not be able to indicate the pregnancy outcome of IVT-ET. There are high embryo implantation rates and clinical pregnancy rates but the dosage of gonadotropin administration is increased in low serum LH level group.
