狼疮肾炎患者T淋巴细胞不同凋亡状态的意义

【摘要】 目的探讨狼疮肾炎(LN)活动期与静止期外周血单个核细胞(PBMC)凋亡率、CD3+、CD3+CD4+、CD3+CD8+、CD3+CD4+/Fas、CD3+CD8+Fas、sFas表达的意义。方法56例经肾活检证实的LN,其中活动期38例,静止期18例,均采用酶联免疫吸附试验(ELISA)检测sFas、白细胞介素(IL)-1"和流式细胞仪检测PBMC凋亡率、CD3+、CD3+CD4+、CD3+CD8+、CD3+CD4+Fas、CD3+CD8+Fas指标。结果LN活动期PBMC凋亡率、sFas明显升高;静止期sFas较正常组增高(P<0.01);CD3+细胞比正常组增多(P<0.05)、CD3+CD4+细胞较正常组明显减少(P<0.01)、CD3+CD8+细胞比正常组增多(P<0.01);PBMC凋亡率与sFas呈正相关(r=0.523);活动期、静止期CD3+CD4+、CD3+CD8+、FasL表达极少。结论T细胞亚群的不同凋亡状态在LN的发生发展中具有重大意义,细胞凋亡增加与血清sFas水平相关,sFas可能抑制肾小球系膜细胞凋亡,而且可能是LN进展中的一个因素。

【Abstract】 Objective To investigate the apoptosis level of peripheral blood monocytes cell (PBMC), CD3+, CD3+CD4+, CD3+CD8+, CD3+CD4+Fas, CD3+CD8+ Fas, sFas in lupus nephritis (LN) patients in the active stage and remission stage. Methods Fifty-six LN cases were recruited in this study, in which 38 cases were in active stage, and 18 cases were in the remission stage. The leves of sFas, IL-1?茁 by ELISA, and the apoptosis level of PBMC, CD3+, CD3+CD4+, CD3+CD8+, CD3+CD4+ Fas, CD3+CD8+ Fas were examined by flow cytometry. Results Compared with the normal, the apoptosis level of PBMC, the expression of sFas, CD3+CD4+Fas and CD3+CD8+ cells were increased significantly in the active stage (P<0.01) and the CD3+ cells were also increased (P<0.05). In the remission stage, the expression of sFas, CD3+CD4+Fas, CD3+CD8+ cells and CD3+CD8+ Fas were increased significantly (P<0.01). Compared with the remission stage, the expression of sFas was increased significantly in active stage (P<0.01). There was positive correlation in the apoptosis level of PBMC and the expression of sFas (r=0.523). The expression of CD3+CD4+, CD3+CD8+, sFasL in both active stage and remission stage were very few. Conclusion In the active stage of LN, the apoptosis level of PBMC is higher than normal. The increased sFas may inhibit the apoptosis of glomerular mesangial cells (GMC) and it may play an important role in the development of LN.
