经亚低温治疗的重型脑损伤患者外周血NSE的动态变化及临床意义

【摘要】 目的研究经亚低温治疗的重型脑损伤患者其外周血神经元特异性烯醇化酶(NSE)的动态变化及其临床意义。方法52例重型脑损伤患者随机分为亚低温组和对照组,前者进行亚低温治疗,后者除了不进行亚低温治疗外其他治疗方法与前者相同,分别于手术的次日、第2、3、5、7天测其外周血的NSE,满3个月进行GOS预后评分。结果发现经过一定时间的亚低温治疗,亚低温组外周血的NSE与对照组比较有明显的差异,满3个月进行GOS预后评分,亚低温组明显优于对照组(P<0.05)。结论亚低温的确是治疗重型颅脑损伤的有效手段之一,其外周血NSE的动态变化能够准确反映治疗效果,而且简便易行,是评价重型脑损伤患者伤情及预后的客观指标之一。

【Abstract】 Objective To study the dynamic changes and clinical significance of neuron-specific enolase(NSE)in peripheral blood of severe brain injury patients treated by mild hypothermia.Methods A total of 52 severe brain injury patients were randomly assigned to the treatment group(n=26)and the control group(n=26).Besides the routine treatment,mild hypothermia was used in the treatment group and their temperature were maintained at 32~34℃for 7 d.The control group received routine treatment only. NSE and blood glucose were measured in the 2 groups before treatment and 2,3,5,7 d after treatment. Prognosis was evaluated with GOS at 3 months.Results NSE,in the treatment group were lower than those in the control group after treatment;there was significant difference between the 2 groups.GOS of treatment group showed a significant improvement as compared with that of control group at 3 months (P<0.05).Conclusion Mild hypothermia is one of effective means of treating the patients with severe brain injury.The level of NSE in peripheral blood can reflect the results of treatment accurately and simply, it can,therefore,serve as one of objective indexes for appraising the traumatic degree and prognosis of patients with severe brain injury.
