胃癌组织中CagA~+幽门螺杆菌感染和环氧合酶-2检测

【摘要】 目的:探讨胃癌组织中CagA+Hpylori感染对环加氧酶2(cox2)mRNA表达的影响。方法:采用原位杂 交法检测40例慢性浅表性胃炎、31例慢性萎缩性胃炎伴肠化、34例不典型增生、55例胃癌组织中cox 2mRNA的 表达情况;快速尿素酶试验和warthin starry银染色结合C14 呼气试验检测Hpylori感染情况;斑点金免疫渗滤法检 测患者血清抗HpyloriCagAIgG抗体。结果:慢性浅表性胃炎、慢性萎缩性胃炎伴肠化、不典型增生和胃癌组织中 cox 2mRNA的表达率呈递增趋势,胃癌与慢性浅表性胃炎组织中的cox 2mRNA表达率差异有统计学意义(P< 0.05);cox 2mRNA表达与胃癌的分化程度及淋巴结转移情况有关(P<0.05);Hpylori阳性慢性浅表性胃炎和慢 性萎缩性胃炎伴肠化组织中cox 2mRNA阳性表达率高于Hpylori阴性组,差异有统计学意义(P<0.05);在不典 型增生和胃癌组织中Hpylori阳性组与Hpylori阴性组cox 2mRNA的阳性表达率差异无统计学意义(P>0.05);H pylori阳性慢性浅表性胃组织炎中抗HpyloriCagAIgG抗体阳性率低于Hpylori阳性胃癌组织,差异有统计学意义 (P<0.05);cox 2mRNA表达与HpyloriCagA密切相关(P<0.05)。结论:Cox 2参与胃癌的发生发展,同时也增 加其转移的潜能;Hpylori通过诱导cox 2表达上调参与胃癌病变;CagA是Hpy

【Abstract】 Aim: To investigate the influence of CagA + H pylori infection on cox-2 mRNA expression in gastric carcinoma tissue. Methods: In situ hybridization was used to detect the expression of cox-2 mRNA in 40 cases of chronic supperficial gastritis (CSG), 31 cases of chronic atrophic gastritis with intestinal metaplasia(IM), 34 cases of dysplasia (DYS), and 55 cases of gastric carcinoma(GC). Rapid urease test, warthin-starry dyer and С 14-urea breath test were used to detect H pylori. Dot immunogold filtration assay was used to detect H pylori CagA IgG antibody. Results: The positive rate of cox-2 mRNA expression increased with the deterioration of gastric lesions.Expression of cox-2 mRNA was related to differentiation and lymph node metastasis of GC (P<0.05).The positive rate of cox-2 mRNA expression in H pylori-positive CSG and IM was significantly higher than that of H pylori-negative CSG and IM (P<0.05). Between H pylori-positive and H pylori-negative groups, there were no significant differences in the positive rate of cox-2 mRNA expression in DYS and GC (P>0.05). The positive rate of H pylori CagA IgG antibody in H pylori-positive GC patients was significantly higher than that of H pylori-positive CSG patients(P<0.05). cox-2 mRNA expression was positively correlated with H pylori CagA + (P<0.05). Conclusion: Cox-2 may play a role in human gastric carcinogenesis.H pylori infection could stimulate the expression of cox-2, which might promote gastric carcinogenesis.
