儿童过敏性紫癜血浆一氧化氮与内皮素-1变化的意义

【摘要】 目的 :探讨内源性一氧化氮 (NO)和内皮素 - 1(ET- 1)在儿童过敏性紫癜 (HSP)发病中的作用。方法 :对4 8例 HSP患儿 [紫癜性肾炎 (HSPN)组 2 1例 ,非肾脏受累 (NHSPN)组 2 7例 ]急性期、恢复期血浆 NO、ET- 1水平进行测定。结果 :HSP患儿急性期血浆 NO、ET- 1水平均明显高于恢复期及对照组 (P<0 .0 1) ,尤以 HSPN组升高更显著 ,与 NHSPN组比较差异有显著性 (P<0 .0 5 ) ;急性期血压升高组 NO/ ET- 1比值较血压正常组低(P<0 .0 5 ) ;恢复期非肾脏受累组 NO、ET- 1水平虽仍较对照组高 ,但差异无显著性 (P>0 .0 5 ) ;HSPN组恢复期NO、 ET- 1水平虽较急性期下降 ,但仍明显高于对照组 (P<0 .0 5 ) ,且 NO/ ET- 1比值小于急性期 ;HSPN组和NHSPN组急性期和恢复期 NO与 ET- 1呈明显正相关 (HSPN组急性期 r=0 .784 3,P<0 .0 1,恢复期 r=0 .4 92 0 ,P<0 .0 1;NHSPN组急性期 r=0 .6 831,P<0 .0 1,恢复期 r=0 .6 317,P<0 .0 1)。结论 :NO、ET- 1的水平可作为临床判定 HSP患儿病情轻重、并发症的发生及预后的重要指标。

【Abstract】 Objective To investigate the effects of nitric oxide(NO) and endothelin-1(ET-1) on the children with Henoch-Schonlein′s purpura(HSP). Methods The plasma NO and ET-1 levels of 48 children with HSP,inclnding 21 children with HSP nephritis (HSPN) and 27 children with non-HSPN (NHSPN), were determined both in acute and recovery phases. Results The concentrations of the plasma NO and ET-1 in acute phase were much higher than those in recovery phase and control group, respectively (P<0.01), and the changes were much more significant in children with HSPN than those without HSPN. The ratio of NO/ET-1 of hypertension group was lower than that of children without hypertension in acute phase (P<0.05). There were no significant differences of the plasma NO and ET-1 between NHSPN group in recovery phase and the control group(P>0.05). However, the plasma NO and ET-1 concentrations of the HSPN group in recovery phase were still higher than those in the control group (P<0.05),and were lower than acute phase, and the ratio of NO/ET-1 in recovery phase was lower than that in acute phase. There was a positive correlation between NO and ET-1 in children with HSP (HSPN group in acute phase:r=0.7843,P<0.01,in recovery phase:r=0.4920,P<0.01);NHSPN group in acute phase r=0.6831,P<0.01,in recovery phase r=0.0.6317,P<0.01).Conclusion The levels of NO and ET-1 are important indexes to determine the state of illness, occurrence of complication and prognosis in children with HSP.
