【摘要】 目的分析总结难治性癫痫的临床特点 ,讨论如何改进手术方法减少手术死亡率。方法　回顾性分析 4年来经显微手术治疗的 2 8例难治性癫痫病例 ,其中 13例采用联合手术方法 ,所有病人均在皮层脑电图监测下进行。结果　所有病人随访 6个月至 4年 ,癫痫发作完全控制 18例 ,显著改善 8例 ,无变化 2例 ,无明显并发症 ,无死亡。结论　癫痫手术治疗不仅能控制发作、减少抗癫痫药的副反应 ,还可以改善难治性癫痫病人的生活质量 ,远期疗效还有待更多的病例积累和长期跟踪观察。更多还原

【Abstract】 Objective To study the clinical features of intractabal epilepsy (IE),and discus how to improve surgical skill and reduce the operative mortality. Methods 28 cases of IE treated by microsurgery in last 4 years were analyzed retrospectively, and among of them,13 had undergone combined operations.All operations were performed with the guidance of intraoperative electrocorticography. Results All the patients were followed-up for 6 months to 4 years. 18 patients got seizure completely disappeared (Engel’s class Ⅰ), 8 patients with the seizure frequency decreased significantly (Engel’s class Ⅱand Ⅲ), and 2 patients remained unchanged (Engel’s class Ⅳ). No morbidity or mortality was observed in all patients.Conclusions Epilepsy surgery can not only control seizure, but also decrease the side-effects of anti-epilepsy drugs and improve quality of life of IE patients. These experiences need further confirmation by larger series and long-term follow-up.更多还原
