胆胰液脱落细胞DNA倍体分析在诊断胆道和胰腺良恶性疾病中的价值

【摘要】 目的 探讨胆胰液脱落细胞DNA倍体分析对胆道腺胰良恶性疾病的诊断价值。方法 用流式细胞术对42例胆道或胰腺癌患者和30例良性胆道或胰腺良性疾病患者的胆胰液中脱落细胞行DNA倍体分析,同时将DNA倍体分析、B超、计算机断层扫描(CT)、内镜逆行胰胆管造影(ERCP)结果分别与胆胰疾病手术和病理确诊结果进行对比。结果 术前B超、CT、ERCP、DNA倍体分析对胆胰良恶性疾病的诊断正确性分别为56.9%、61.1%、69.4%和84.7%,阳性预测值分别为72.0%、76.9%、77.8%和91.9%,阴性预测值分别为48.90%、52.1%、61.1%和77.1%。DNA倍体分析对胆胰良恶性疾病诊断的敏感性和特异性分别为80.9%和90.O%。结论 DNA倍体分析对胆道胰腺疾病的良恶性诊断价值显著优于B超、CT和ERCP,可能是一项有价值的临床诊断指标。

【Abstract】 Objective To explore the diagnostic value of DNA doublet analysis of falling off cells from cholangiopancreatic drainage in the diseases of bile and pancreas. Methods DNA doublet analysis of falling off cells from cholangiopancreatic drainage in 72 cases was performed with flow cytometry. The results were compared with those of B-mode ultrasonography, CT and ERCP according to the final diagnosis. Results The validity (general index reflecting sensitivity and specificity) for diagnosing in B-mode ultrasonography, CT, ERCP and DNA doulet analysis were 56. 9% , 61. 1% , 69. 4% and 84. 7% respectively. The positive predictive value were 72.0%, 76.9%, 77.8%, 91.9% and the negative predictive value were 48. 9% , 52. 1% , 61. 1% , 77. 1% respectively. The sensitivity and specificity of DNA doublet analysis for the diagnosis of diseases of bile and pancreas were 80. 9% and 90.0%. Conclusion DNA doublet analysis had greater value than B-mode ultrasonography, CT and ERCP to differentiate the malignant diseases of bile and pancreas from benign ones. It might be a valuable diagnostic method.
