【摘要】 目的:探讨Vojta姿势反射检查在小儿脑瘫早期的诊断价值。方法:应用Vojta姿势反射检查法对有围产期高危因素的12个月以内婴儿及脑瘫患儿共160例进行神经发育检查及相关因素调查,并对结果进行统计学分析。结果:160例婴儿中Vojta姿势反射检查95例异常,疑诊为脑瘫95例,临床诊断脑瘫86例,Vojta姿势反射检查法与临床对脑瘫的诊断符合率为91.86%(79/86),两者相关性检验X2=81.352,P<0。001;配对X2检验,P>0.05。结论:Vojta姿势反射检查诊断早期脑瘫敏感性高,与临床诊断符合率为91.86％。Vojta姿势反射检查结合运动发育检查是目前诊断早期脑瘫较为准确而敏感的方法之一,在早期脑瘫筛查工作中具有重要的诊断价值,若能结合头颅CT或MRI等辅助检查,将极大提高早期诊断准确性,在儿童保健门诊开展神经发育筛查工作对早期发现、早期干预具有可行性及重要意义。更多还原

【Abstract】 To explore the diagnostic value of Vojta posture reflex in infants with early cerebral palsy. Methods: The nerve growth check and correlated factors of 160 cases of infants in 12 months with perinatal high risk factors and children with cerebral palsy were conducted by Vojta posture reflex. The results were analyzed statistically. Results:Of 160 cases, 95 cases with abnormal Vojta posture reflex were suspected of cerebral palsy, 86 cases were diagnosed as cerebral palsy clinically. The accordant rate between Vojta posture reflex examination and clinical diagnosis was 91.86% (79/86). Correlation test was X2= 81.352, P<0. 001; matched pairs X2test, P>0. 05. Conclusion: Vojta posture reflex examination had high sensitivity in early diagnosis of cerebral palsy, the accordant rate was 91.86% to clinical diagnosis. Vojta posture reflex examination combining movement growth examination were one of accurate and sensitive methods for early cerebral palsy diagnosis, which had important diagnostic value. If it can be combined with skull CT or MRI, the early diagnostic accuracy could be improved greatly. Developing nerve growth screening work in clinic of children healthcare has feasibility and important significance in early detection and intervention of this disease.更多还原
