【摘要】 目的　观察经瞳孔温热疗法 (TTT)对渗出型老年性黄斑变性 (AMD )的临床效果。方法　对 19例荧光素眼底血管造影 (FFA)和吲哚菁绿血管造影 (ICGA)检查确诊为AMD的患者的 2 2只患眼进行TTT治疗。采用 810nm半导体激光 ,光斑 0 5～ 3 0mm ,能量 2 0 0～ 40 0mW ,照射时间 60s。对三面镜眼底检查、FFA、ICGA图像所显示的脉络膜新生血管 (CNV)进行照射 ,照射区未出现颜色变化或呈淡灰白色。对比分析患者治疗前后视力、眼底及OCT检查。结果　 2 2患眼治疗后l～ 10个月 ,平均随访观察时间 4 8个月 ,视力不变者 13例 ,占 5 9 1% ;进步者 7例 ,占 3 1 8% ;下降者 2例 ,占 9 1%。 2 0眼OCT复查者中 ,黄斑水肿不变者 11例 ,占 5 5 % ;好转者 7例 ,占 3 5 % ;恶化者 2例 ,占 10 %。再次行TTT治疗者 5眼 ,占接受TTT治疗者的 2 2 7%。治疗随访期间无明显治疗副作用。结论　TTT治疗能使大部分渗出性AMD患眼视力稳定或提高 ,使用安全 ,值得临床推广应用。更多还原

【Abstract】 ObjectiveTo evaluate the effectiveness of transpupillary thermotherapy(TTT)on age-related macular degeneration(AMD).MethodsTwenty-two eyes(19 cases)suffering from exudative AMD were managed with a diode laser at 810 nm.A variable spot size of 0.5-3 mm was used for 60-second duration at a power ranged 200-400 mW.The fundus fluorescein angiography(FFA)and indocyanine green angiography(ICGA)were performed as a diagnostic index before treatment,and the visual acuities and coherence tomography(OCT)in pre- and post-operation were compared.ResultsNo visible color change or a light-gray appearance was seen in the treated area.The visual acuity was improved in 7 eyes (31.8%),unchanged in 13 eyes(59.1%)and declined in 2 eyes(9.1%).OCT revealed that macular edema was unchanged in 11 eyes(55%),decreased in 7 eyes(35%)and increased in 2 eyes(10%).The secondary treatment was performed in 5 eyes (22.7%).No adverse effect was found.ConclusionTranspupillary thermotherapy (TTT) is a potential method for exudative AMD.更多还原
