DWI对急性多发性脑梗死的诊断价值
DIFFUSION-WEIGHTED IMAGING IN THE DIAGNOSIS OF ACUTE MULTIPLE BRAIN INFARCTION
【摘要】 ①目的 观察磁共振弥散加权像 (DWI)诊断急性多发性脑梗死 (AMBI)是否比常规MRI具有优越性 ,同时对多发性脑梗死的病因及发病部位进行初步探讨。②方法 对 136例临床诊断脑梗死的病人进行常规MRI、MRA及DWI检查 ,必要时行脑血管造影术 (DSA)。③结果 136例脑梗死病人利用DWI技术检出AMBI4 1例 ,而利用常规MRI检出AMBI 2 0例 ,DWI技术比常规MRI具有更高的检出率 (χ2 =18.5 ,P <0 .0 1)。 4 1例AMBI在前循环 2 6例 ,其中一侧大脑半球 14例 (A组 ) ,双侧大脑半球 12例 (B组 ) ;AMBI在后循环 9例 (C组 ) ;前后循环都有 6例 (D组 )。大动脉粥样硬化所致的AMBI在前三组常见 ;心源性栓子在D组更常见。④结论 应用DWI诊断AMBI比传统MRI具有更大的优越性 ;不同解剖部位的AMBI其病因不同 ,DWI结合MRA有助于明确AMBI在不同解剖部位的不同发病机制。
【Abstract】 Objective To compare the effects of diffusion-weighted imaging(DWI) and those of conventional MRI in the detection of acute multiple brain infarction(AMBI).The sites and etiology of the disease were discussed preliminarily. Methods MRI/MRA and DWIwere performed for 136 cerebral infarction patients. DSA was also done when necessary. Results In 136 AMBI cases, 41 were found with DWI and 20 with conventional MRI (χ2=18.5,P<0.01). In the 41 AMBI cases,26 were in anterior circulation,with 14(Group A) in unilateral hemisphere and 12 (Group B) in bilateral hemisphere; nine (Group C) were in posterior circulation; and six(Group D) in both anterior and posterior circulation. AMBI caused by large-artery arteriosclerosis was more commonly seen in groups A, B, and C, and cardio embolism in group D. Conclusion DWI is superior to conventional MRI in the diagnosis of AMBI. The AMBI etiology is different with different sites. The combination of DWI with MRA is helpful in confirming the mechanisms of various locations of AMBI.
