腓浅动脉皮支蒂皮瓣的临床解剖
Microanatomy of The Cutaneous Branches of Superficial Peroneal Artery
【摘要】 目的 观察腓浅动脉皮支血管的数量、发出的位置、血管的直径、长度以及发出后与主干大体的角度 ,为形成腓浅动脉皮支蒂皮瓣提供解剖学基础。方法 采用经股动脉热红明胶灌注的尸体 18具 (36侧 ) ,经月国动脉灌注的下肢标本 4侧 ,共 4 0侧 ,在手术显微镜下对腓浅动脉及其皮支进行解剖和观察。结果 腓浅动脉有 3~ 5支皮支发出 ,前 3支恒定出现 ,第 1支在腓浅动脉刚出肌间隔处 ,距腓骨小头 13.76± 2 .36cm。 3支的血管外径、长度分别为 0 .6 1± 0 .2 5mm和 2 .12± 0 .32cm ,0 .4 5± 0 .2 0mm和 1.6 3± 0 .4 2cm ,0 .4 3± 0 .2 1mm和 1.72± 0 .4 6cm。距第 1支 3.4 4± 0 .98cm发出第 2支 ,距第 2支 3.5 3± 0 .74cm发出第 3支。第 4支出现率 12 .5 % ,血管直径 0 .0 4mm ,长度 1.6 8± 0 .31cm ,距腓骨小头 2 2 .38± 0 .95cm。第 5支出现率 7.5 % ,外径 0 .19± 0 .0 1mm ,长度 1.5 7± 0 .77cm ,距腓骨小头约 2 4 .12± 1.0 6cm。结论 ①浅动脉在出肌间隔及筋膜下段恒定地发出 3~ 5条皮支血管 ;②前 3支出现率高 ,管径及长度差异无显著性 ;③腓浅动脉皮支血管管径较粗 ,可以形成带蒂皮瓣。
【Abstract】 Objective To investigate the branches of superficial personal artery and provide the anatomic basis for the flap based on it.Methods Perfuse the femoral artery in 18 adult cadavers (36sides) and popliteal artery in 4 lower legs with total 40 sides.Dissect and observe the superficial peroneal arteries and branches.Results All the superficial peroneal arteries observed arise from the anterior tibial arteries.The first branch occurs at the superficial peroneal artery just comes out of the intermuscular septum.The second is 3.44±0.98cm away from the first,the third,3.53±0.74cm away from the second,both with an occurrence rate of 100%.The diameters of the 3 branches are 0.61±0.25mm,0.45±0.20mm and 0.43±0.21mm,lengths are 2.12±0.32cm,1.63±0.42cm and 1.72±0.46cm,their distance from the head of fibula,are 13.76±2.36cm,16.58±1.13cm and 20.07±1.27cm.The occurrence rate of the fourth and fifth is 12.5%,7.5%.diameter,lenth and distance to the head of fibula is 0.26±0.04mm,1.68±0.31cm and 22.38±0.95cm;0.19±0.01mm,1.57±0.77cm and 24.12±1.06cm.Conclusion ①There are 3~5 cutaneous arteries starting from the superficial peroneal artery;②For the first 3 branches occurring constantly,there is no statistically significant difference founded in diameters and lengths;③The vessels being coarse,the pedicle flaps based on the artery can be designed.
