【摘要】 目的 :探讨Hb值对临床节约用血的指导作用。方法 :选择 30 0例 (其中外科 2 2 0例、妇科 80例 )术前无输血 ,术中出血且失血量超过血容量的 2 0 %的成人手术输血患者分二组进行研究 ;Ⅰ组 1 5 5例 (其中外科 1 1 3例、妇科 4 2例 )患者术前Hb值 1 0 0g/L以上 ,给予患者术中 1 /3失血量的输血量 ;Ⅱ组 1 4 5例 (其中外科 1 0 7例、妇科 38例 )患者术前Hb值 80～ 1 0 0g/L ,给予患者术中 2 /3失血量的输血量 ;二组 30 0例患者均在充分补液的基础上 ,再给予输血 ,然后再输给晶体液和胶体液补充患者的血容量。观察二组患者术中呼吸、血压、脉率、输血反应情况 ,测量患者术后的Hb值及Hct,记录患者术后Hb值及Hct恢复至术前水平的天数 ,记录患者术后切口愈合期别和时间。结果 :二组患者术中呼吸、血压、脉率均正常 ,无输血反应 ;二组患者术后的Hb值及Hct分别与术前的Hb值及Hct比较差异无显著性 (P >0 .0 5 ) ;二组患者术后Hb值及Hct恢复至术前水平的天数差异无显著性 (P >0 .0 5 ) ;二组患者术后切口愈合期别和时间差异无显著性 (P >0 .0 5 )。结论 :Hb值能指导外科、妇科手术患者的输血 ,对临床节约用血具有重要意义。更多还原

【Abstract】 Objective: To explore the instructive function of Hb value on clinical blood transfusion saving. Method: 300 cases of adopted patients (220 cases from surgical department and 80 from department of gynaecology) without blood transfusion before surgical operation who undergoing blood transfusion due to blood loss during surgical operation were divided into two groups; Patients of Group one (113 from surgical department and 42 from department of gynaecology ) whose Hb was over 100g/L before surgical operation were given one third of their blood loss volume during surgical operation ; Patients in group two (107 from Surgical department and 38 from department of gynaecology ) whose Hb value ranged from 80～100g/L before operation were given two third of their blood loss volume during operation; All patients were given adequate sulotion transfusion before blood transfusion . Recovery time for the Hb. Hct to reach their preoperative standard and recovery time of the surgical cut were recorded. Result: The breath rate, Bp, Pulse rate of all patients were normal, No blood transfusion reaction was recorded; Post-operative Hb and Hct in two groups were not significantly different from the preoperative ones; Recovery time of Hb and Hct and recovery time of the surgical cut of all patients also had no significantly different. Conclusion: Hb value can be use as a guideline on blood transfusion for surgical or gynaecological patients and had great significance for clinical save blood.更多还原
