计算机辅助数字图像分析定量检测肝纤维化的应用研究
A Study of Quantitative Assessment of Hepatic Fibrosis by Using Computer - assisted Digital Image Analysis
【摘要】 [目的]探讨计算机辅助数字图像分析法测量肝纤维化的应用价值。[方法]采用计算机辅助数字图像分析法定量评估116例因各种肝胆疾病手术切除肝组织标本的纤维化程度,并与Ishak半定量分级法进行比较。[结果]数字图像分析法测定116例标本肝纤维化面积百分比为2.5~49.3;Ishak分级0-6级者分别为8、15、46、8、9、15、15例。随着Ishak级数加大,纤维化面积比渐次增大,ANOVA分析示Ishak 0级与1级及1级与2级间纤维化面积比无明显差异(P>0.05),其余任何两个相邻级别间纤维化面积比差异均存在显著性(P<0.01)。两种方法检测结果之间存在显著相关性(相关系数r=0.883,P<0.001)。[结论]数字图像分析法能够比较客观地评估肝纤维化程度,可用于诊断肝纤维化、判断治疗的效果以及术前预测肝储备功能。
【Abstract】 [Objective] To investigate the application of computer-assisted digital image analysis in quantitative assessment of hepatic fibrosis. [ Methods ] Computer-assisted digital image analysis was used to assess the degree of hepatic fibrosis in 116 surgically excised liver specimens due to various hepatobil-iary diseases, and compared with semi-quantitative score. [ Results ] Computer-assisted digital image analysis revealed that fibrosis area ratio in 116 liver specimens was ranged between 2. 5 and 49. 3. The semi-quantitative classification showed that there were 8, 15, 46, 8, 9, 15, 15 cases in score 0, 1, 2, 3, 4, 5, 6, respectively. With increase in Ishak score, the fibrosis area ratio increased in proper order. ANOVA analysis found that there was a significant difference in fibrosis area ratio between any two scores (P < 0. 01), except for score 0 and 1, and 1 and 2 ( P > 0. 05) . Correlation analysis demonstrated that a close correlation existed between two methods, with a correlation coefficient r of 0. 883 (P< 0. 001). [Conclusion] Computer-assisted digital image analysis represents an objective method for assessing hepatic fibrosis. It may be useful in diagnosing hepatic fibrosis, evaluating therapeutic efficacy for hepatic fibrosis and predicting hepatic functional reserve preoperatively.
