下丘脑-垂体-甲状腺轴内分泌激素变化对急性脑梗死病因诊断的意义

【摘要】 目的 探讨下丘脑-垂体-甲状腺轴内分泌激素在急性脑梗死病因中的作用。方法 研究对象按是否患脑梗死和是否存在脑梗死危险因素共分3组:脑梗死组、脑梗死高危组,正常对照组。用放射免疫法检测血清中三碘甲状腺原氨酸(T3)、甲状腺素(T4)、促甲状腺素(TSH)、抗甲状腺球蛋白抗体(TGA)、抗甲状腺微粒抗体(TMA)水平,用独立样本τ检验比较各组间差异。结果 脑梗死组与脑梗死高危组和正常对照组相比,T3、T4水平显著下降;TSH水平明显升高,差异有显著性;脑梗死高危组与正常对照组相比,T3水平明显升高,有显著性差异,T4水平呈升高的趋势,但无显著性差异;脑梗死高危组中血清TSH水平较正常对照组低,但无显著性差异。结论 本研究提示脑梗死患者及脑梗死高危人群中存在下丘脑-垂体-甲状腺轴功能异常;下丘脑-垂体-甲状腺轴功能异常可能是脑梗死的独立危险因素之一。

【Abstract】 Objective In order to explore the effect of hypothalamus-pituitary-hypothyroid axis on acute cerebral infarction onset. Methods Objects who were studied were divided into 3 groups: cerebral infarction group (GIG) 、 persons with cerebral infarction risk factor group (CIRG) and control group (CG). The concentration of serum T3、T4、TSH、TGA and TMA in each group were measured respectively by radioimmunoassay, and results were compared between each two groups by using Student t test. Results Compoared with CIRG and CG, the levels of T3、T4 in the sample of CIG were olwer, and the level of TSH were higher, and there aws distinctly difference ( P < 0.01) ; The level of T3 in CIRG was higher than that of CG, and there was distinctly difference (P < 0.01); Otherwise, Compared with CG, the level of T4 was higher and the level of TSH was lower in CIRG, but there was no distinctly difference between CIRG and CG ( P > 0.01). Conclusions This study indicated that the function of hy-pothalamus-pituiary-hypothyroid axis was abnormity in patients with cerebral infarction and those persons with cerebral infarctive risk factor, and that the function of hypothalamus-pituitary-hypothyroid axis was abnormity probably aws an important risk factor in cerebral infarction onset
