【摘要】 [目的]评价原发性骨肿瘤全身放射性核素骨显像的临床价值。[方法]84例原发性骨肿瘤患者行99Tcm鄄亚甲基二膦酸盐(MDP)全身骨显像,观察原发性骨肿瘤的显像特征,并对术前远处转移情况、术后复发和转移情况进行分析,部分患者联合应用99Tcm(V)鄄二巯基丁二酸钠(DMSA)肿瘤阳性显像判断是否复发。[结果]84例原发性骨肿瘤单发病灶的99Tcm鄄MDP显像特征为病灶部位的高摄取,在放射性浓集的热区病灶中可见大小不等的减低区,伴或不伴有软组织浓集。29例HIFU治疗前病灶部位呈异常放射性浓聚,治疗后呈放射性减低或缺损,病灶范围缩小;有部分病例治疗后病灶周围正常骨组织放射性较治疗前增高。[结论]对原发性骨肿瘤,99Tcm鄄MDP全身骨显像可灵敏地显示原发病灶,检测是否出现多处病灶,有无其他部位骨骼转移,且可供评估骨肿瘤活性,并可用于治疗后疗效评估和监测复发及转移,联合应用99Tcm(V)鄄DMSA阳性显像可提高诊断的特异性。更多还原

【Abstract】 To ev aluate the clinical value of whole-body bone scintigraphy in the diagnosis for primary malignant bone tumor.Eighty-four patients with primary maligna nt bone tumor were administered by bone scan with 99 Tc m-methylene diphosphonate(MDP).99 Tc m (V)-DMSA scintimammography was applied for evaluating recurrence in some patients.The solitay primary lesion shows high radioactivity in imaging with lower-or non-radioactivity areas among the lesion.The high uptaking of 99 Tc m-MDP was presented in29cases before the HI-FU treatment,while after treatm ent lower uptake or no uptake areas of the radiotracer at the same site was foun d,and the volum of lesion was decreased.The images of ring or semiring like high uptake of the radiotracer around the focus were found in some patients.The upta ke of 99 Tc m (V)-DMSA in the lesion in some patients indicated recurrent tumor.[Conclu sion] 99 Tc m-MDP whole-body scans is a sensitive technique for detecting the primary l esion and metas-tasis,evaluating the status of the tumor.Furthermore it can eva luate the HIFU effect and find recurrence and meatastasis.Scintimammography with 99 Tc m (V)-DMSA can improve the specificity in diagnosis.更多还原
