【摘要】 [目的]探讨血管和淋巴管癌栓与宫颈癌术后治疗和预后的关系。[方法]回顾性分析512例宫颈癌根治术后病理切片,发现31例血管癌栓和48例淋巴癌栓,应用卡方检验和SPSS10.0软件分析血管及淋巴管癌栓与淋巴结转移等病理特征的相关性及其对预后的影响。[结果]宫颈癌患者血管和淋巴管癌栓的发生与患者年龄、组织类型无明确的相关性(P>0.05),血管和淋巴癌栓与肿瘤的浸润深度、分化程度相关(P值分别<0.001和<0.01)。淋巴癌栓组淋巴结转移率显著高于无癌栓组(P<0.05),血管和淋巴癌栓组的复发和/或转移率高于无癌栓组(P<0.001),血管癌栓组和无血管癌栓组5年生存率有明显差异分别为54.84%与89.22%(P<0.001),淋巴癌栓组与同期无淋巴癌栓组5年生存率也有明显差异,分别为64.58%与89.49%(P<0.001)。[结论]血管和淋巴癌栓是宫颈癌患者的重要预后因素。更多还原

【Abstract】 To explore the relationship between vessel cancer embolu s and prognosis in patients with cervical carcinoma.Five hundred and twelv e patients with cervical carcinoma treated with radical hysterectomy were analys ed retrospectively.Blood vessel embolus in31patients and lymph vessel embolus in 48patients were found.The correlation among blood vessel or lymph vessel cancer embolus and lymph node metastasis,and other histopathologic factors was investig ated by Chi-square analysis.The influence of blood vessel and lymph vessel canc er embolus on survival was analysed with SSPS10.0soft-ware.The inciden ce of vessel cancer embolus was related to grade and depth of invasion.The lymph node metastasis occurred frequencently in the patients with vessel cancer embol us.More cancer re-currence and metastasis were found in vessel cancer embolus g roup cempared with no vessel cancer embo-lus group.Five-year survival in blood vessel with and without cancer embolus was54.84%and89.22%,re-spectively(P<0.0 01),and that in lymphatic vessel with and without cancer embolus was64.58%and89. 49%,respectively(P<0.001).[Conclusion]Vessel cancer embolus are important progn ostic factor in the patients with cervical carcinoma.更多还原
