苍白球内侧部运动觉细胞空间位置测定及其意义
A study on the topographic localization of kinesthetic cells in the interior segment of globus pallidus and its clinical importance
【摘要】 目的 研究苍白球内侧部运动觉细胞空间位置排列及其在苍白球毁损术中的意义。方法 对 34例病人苍白球内侧部运动觉细胞进行了空间位置测定 ,每例病人测定 5~ 1 3个部位 ,共记录 32 2个部位。结果 82(2 5 .5 % )个细胞随对侧肢体关节被动运动其放电频率有变化 ,其变化形式为放电频率或幅度的增加及减少。上肢的运动觉细胞主要集中在GPi的背侧和腹侧 ,而下肢的运动觉细胞更靠近GPi的中心部位。结论 上肢和下肢的运动觉细胞在GPi内有很明确的空间定位 ,为合理选择毁损部位提供了依据
【Abstract】 Objective To study the topographic localization of kinesthetic cells in the interior segment of globus pallidus(GPi) and its significance for pallidotomy.Methods Topographic localization of kinesthetic cells of GPi in 34 patients with Parkinson’s disease was studied.Five to 13 neurons were isolated for each patient and studied for movement-related activity.A total of 322 cells were analyzed.Results Of the 322 neurons, 82 (25.5%) changed their firing rate in response to the passive movements of contralateral limbs and joints.The way of response to passive movements of contralateral limbs for the kinesthetic cells was to increase or decrease their firing rate.The majority of arm kinesthetic cells were clustered at the rostral and the caudal segment of GPi.At the same time, the leg kinesthetic cells were located centrally.Conclusion Arm and leg kinesthetic cells had a clear somatotopic organization in Gpi.These results provided information which we choose a correct target reasonably based on in the surgical treatment of Parkinson’s disease.
