【摘要】 目的　了解结核和恶性胸水中可溶性白介素 2受体 (sIL 2R)与肿瘤坏死因子 (TNF)水平及其临床意义。方法　采用双抗体夹心ELISA法和放射免疫分析法 (RIA法 )检测 2 6例结核性、2 2例恶性胸腔积液中sIL 2R与TNF浓度。结果　结核性胸水中sIL 2R与TNF含量分别为 (1310 .4 0± 36 7.89)U ml,(1.79± 0 .5 8) μg L ;恶性胸水中sIL 2R与TNF含量分别为 (94 3.2 6± 2 17.74 )U ml,(1.2 9± 0 .5 1) μg L ;结核性胸水组sIL 2R与TNF浓度均明显高于恶性胸水组 ,两组间有显著性差异 (P <0 .0 0 1,P <0 .0 0 5 ) ;联合检测二者诊断结核性胸腔积液的敏感度、特异度、正确度分别为 10 0 %、96 %、98%。结论　检测胸水中sIL 2R与TNF含量有助于结核性和恶性胸腔积液的鉴别更多还原

【Abstract】 Objective To measure levels of soluble interleukin 2 receptor(sIL 2R) and tumor necrosis factor (TNF) in tuberculous and malignant pleural effusions and the clinical significance.Methods Levels of sIL 2R and TNF expression were tested by ELISA and radioimmunoassay(RIA).Twenty six patients with tuberculous pleural effusions and twenty two patients with malignant pleural effusions were compared.Results sIL 2R and TNF levels were(1 310.40±367.89)U/ml and (1.79±0.58)μg/L respectively in tuberculous pleural effusions,and were(943.26±217.74)U/ml and (1.29±0.51)μg/L respectively in malignant pleural effusions.Concentration of sIL 2R and TNF were significantly higher in tuberculous effusions than that in malignant effusions( P <0.001).The sensitivity,specificity and accuracy of TNF and sIL 2R in the diagnosis of tuberculous pleurisy were 100%,96% and 98% respectively.Conclusion The determination of sIL 2R and TNF levels in the pleural effusions may be contributory to identification of tuberculous and malignant pleural effusions.更多还原
