浙江省基本消除丝虫病后的监测
SURVEILLANCE AFTER FILARIASIS WAS BASICALLY ELIMINATED IN ZHEJIANG PROVINCE
【摘要】 目的 巩固基本消除丝虫病的成果 ,了解基本消除丝虫病后的流行动态和传播规律 ,为消除丝虫病提供科学依据。 方法 在 6 5个原丝虫病流行县开展病原学、蚊媒、血清学纵向和横向监测。 结果 6 5个县 (市 )连续 13~ 2 3年的横向监测结果显示 ,已连续 7~ 2 3年未发现微丝蚴阳性者 ,蚊媒监测 9~ 2 3年无人体幼丝虫感染。 6 5个县 (市 )中有 41个县 (市 )基本消除丝虫病后 10年内的监测乡和人口覆盖率分别为 43.6 5 %和 5 .0 8% ,10年后再监测的乡和人口覆盖率分别为 15 .96 %和 1.2 5 % ;2 4个县 (市 )基本消除丝虫病后 10年以上病原学监测乡和人口覆盖率分别为 39.14%和3.94%。 11个纵向监测点观察 13~ 2 0年 ,发现的 14例残存微丝蚴血症者 ,在未采取治疗措施的情况下 ,均于次年转阴。血清学 (IFAT)监测人群抗体水平已和非流行区人群相近。 结论 浙江省丝虫病传播已被阻断 ,已实现全省消除丝虫病。
【Abstract】 Objective To observe the dynamics of prevalence and the regularity of transmission after filariasis has been basically eliminated and to provide scientific evidence for the elimination. Methods A cross-sectional and longitudinal survey by using parasitological, entomological and serological methods were extensively carried out from 1997 to 2000 in 65 counties previously endemic for filariasis. Results In 65 counties previously endemic for filariasis, cross-sectional surveillance on the disease has been executed for 13-23 years. The results showed that microfilaria cases had not been found for 7-23 years, and filaria larvae had failed to detect from mosquito vectors for 9-23 years. In 41 of 65 counties, the surveillance covering rates of township, population were 43.65% and 5.08 %, respectively in the first ten years, whereas in the following ten years the covering rates were 15.96% and 1.25%. For the more ten years parasitological surveillance of 24 counties where the disease had been basically eliminated, the covering rates of township, population were 39.14% and 3.94% respectively. The results of the 13-20 years observation of 11 longitudinal surveillance stations demonstrated that 14 microfilaremia cases could convert negatively next year with no treatment. There was no significant difference between the filarial antibody levels of inhabitants both living in endemic area and in non-endemic area. Conclusion The transmission of filariasis in Zhejiang has been interrupted and filariasis has been eliminated.
