【摘要】 目的　探讨带血管蒂髂骨移植修复髋臼陈旧性骨折和缺损的治疗方法。方法　将骨折的髋臼复位 ,保留较大的臼顶缘骨块 ,去除其他游离骨碎片 ,切取带旋股外侧动静脉升支为血管蒂的髂骨块固定骨折并修复臼顶和后壁的缺损。术后下肢骨牵引 3周 ,关节和肌肉训练 3周 ,而后扶拐行走锻炼。结果　本组 9例 ,随访 2～ 4年 ,优 4例 ,良 5例 ,3个月后均弃拐行走 ,髋臼愈合良好 ,无股骨头再脱位发生。 1例轻度跛行 ,2例长距离行走时髋关节轻度疼痛。结论　以带旋股外侧动静脉为血管蒂的髂骨块修复髋臼陈旧性骨折和缺损的方法容易操作、效果良好更多还原

【Abstract】 Objective To study the treatment of old fracture of acetabulum and bone defect with vascular pedicled iliac bone grafting Methods 9 cases of old fracture of acetabulum and bone defect were treated.After reduction of fracture,the bigger fracture fragments on the roof of acetabulum were reserved and other free pieces were evacuated.The iliac bone piece with ascending limb of arteria and venae circumflexa femoris medialis was procured and used to fix fracture and repair of the roof and posterior wall of the acetabulum.After the operation,the lower limb was put into traction for 3 weeks and training of the the joints and muscles was instuted for 3 weeks,then the patient was allowed ambulatory with the help of crutches.Results The follow up period ranged from 2 to 4 years.Excellent results were obtained in 4 cases and good in 5.Starting from the 3 rd .month,the patients can walk without crutches and the fracture union was satisfactory.No redislocation of femoral head occurred.Slight limp was observed in 1 case and 2 cases felt slight pain at hip joint after long distance walking.Conclusion:Vascular pedicled iliac bone grafting is an effective method for the treatment of old fracture of acetabulum with bone defect.更多还原
