【摘要】 目的　通过对不同类型儿童慢性鼻窦炎采用不同治疗方式的疗效观察 ,探讨最佳治疗方法。方法6～ 13岁鼻窦炎 45例 ,分为A组 :综合药物治疗Ⅰ型鼻窦炎 2 0例 40侧 ;B组 :腺样体或 /和扁桃体切除 +综合药物治疗Ⅰ型鼻窦炎 14例 2 8侧 ;C组 :内镜鼻窦手术 +综合药物治疗Ⅱ型鼻窦炎 11例 17侧。结果　全部病例随访12～ 2 4个月 ,平均 16个月。A组 40侧 ,治愈 32侧 ,好转 6侧 ,无效 2侧 ;B组 2 8侧 ,治愈 2 4侧 ,好转 2侧 ,无效 2侧 ;C组 17侧 ,治愈 14侧 ,好转 3侧。结论　儿童Ⅰ型鼻窦炎应首选综合药物治疗 ,伴腺样体或 /和扁桃体病变的应在综合药物治疗的基础上切除腺样体或 /和扁桃体 ,Ⅱ型鼻窦炎应在术前、术后综合药物治疗的基础上加行内镜鼻窦手术更多还原

【Abstract】 Objective To determine the optimal treatment of chronic sinusitis in children by different therapies for chronic sinusitis. Methods Forty-five patients (aged 6～13 years) were divided into group A, B and C. In group A, 20 cases (40 sides) were type Ⅰ, treated systemically with medicine. In group B, 14 cases (28 sides) were type Ⅰ and followed by adenoid vegetation and/or chronic tonsillitis, treated systemically with medicine and combined with adenoidectomy and/or tonsillectomy; In group C, 11 cases (17 sides) were type Ⅱ, treated systemically with medicine and endoscopic sinus surgery. Results All patients were followed up for 12～24 months, with an average of 16 months. In group A, 32 sides were cured, 6 sides improved and 2 sides showed no effect. In group B, 24 sides were cured, 2 sides improved and 2 sides had no effect. In group C, 14 sides were cured and 3 sides improved. Conclusion Systemic medinal treatment should be the first choice for type Ⅰ chronic sinusitis in children. Children with adenoid vegetation and/or chronic tonsillitis are basically treated with systemic medicine combined with adenoidectomy and/or tonsillectomy. Administration of medicine before and after the endoscopic surgery is more efficient for type Ⅱ chronic sinusitis in children.更多还原
