【摘要】 目的分析人工髋关节置换假体周围骨折发生的原因,观察记忆合金环抱器治疗假体周围股骨骨折的疗效。方法采用记忆合金环抱器对9例人工髋关节置换术中及术后发生的假体周围股骨骨折进行内固定治疗。全髋术后发生骨折有6例,全髋术中发生骨折有3例。结果9例均获随访,平均随访17.6月(12～19月)。术后1年,除1例翻修术结合环抱器固定有屈髋受限外(45°),其余髋关节屈曲度在85°～95°。结论人工髋关节假体周围骨折的发生,主要与老龄人骨质疏松,骨溶解导致的骨机械强度下降有关;翻修手术操作不当,增加了这类骨折术中发生的机会;记忆合金环抱器内固定为这类骨折的治疗提供了一经济、实用的手段。更多还原

【Abstract】 Objective To analysis the causes resulting in f ractures around the prosthesis of hi p and observe the clinical outcomes o f the embracing fixator for treatment of f ractures around the femoral prosthesis.Methods Shape memory sawteeth-arm embracin g internal fixator was used to treat 9cases of femoral fr actures occurred in or after operati on of THR.There were 6cases who suffe red from fracture after THR .There are 3cases who suffered fr om femoral fractures during total hi p revision.Results The average follow-up period of all c ases was17.6months(12～19months).One year after operation,one patie nt treated with total hip revision combined with embracing fixator had a limited hip flexion(45degrees),the others made an good function of t he hip(85～95degrees).Conclusion The factors of fracture around prosthesis of hip may be relative to o steoporosis in old people,osteolysis near the prosthesis resulting in t he bone mechanical strength de-crease.Unfamiliar with total hip re vision may be increasing the chance t o fracture.The embracing fixator pr ovided a reliable,economical,and practical alternative method for tr eatment of this kind fracture.更多还原
