不同体位下肩袖和肩峰-肱骨头之间距离的MRI研究

【摘要】 目的:分析不同体位时肩袖和肩峰下空间的距离及位置关系。材料和方法:15例正常自愿者,分6个不同体位进行冠状位薄层扫描,观察肩峰外侧点到肱骨头之间的最短距离(A-H)、肩峰下空间和肩袖的位置关系。结果:A-H值在60°~150°外展体位时明显小于0°~30°外展体位时(P<0.01)。肩袖(主要是冈上肌肌腱)在60°~120°时,刚好从中穿过,因而受到肩峰和肱骨头的挤压。0°~30°外展体位时位于肩峰和肱骨头的外侧。150°外展体位时肩袖位于肩峰和肱骨头后内方。结论:60°~120°外展体位时,肩峰和肱骨头之间距离最短,并且肩袖刚好从中穿过,两者关系密切。

【Abstract】 Objective:To explore the distance and the position between acromion and hume rous of normal at different arm abduction.Materials and Methods:Fifteen norm al volunteers were examined with MRI in six arm positions and coronal thin imag es were obtained with a spin echo sequence.Using a special positioning device, the arm was placed at0°,30°,60°,90°,120°,150°arm abducio n.Of them,0°~90°positions were not rotated,while120°and150°pos itions were slight internal rotated.To evaluate not only the minimal distanc e between acromion and humerous(A-H),but also the spatial relationship be- tween the rotator cuff and the subacromial space.Results:The values of A-H at60°~150°arm abducion were obviously smaller than those at0°~30°a rm abducion(P<0.01).But the values of A-H at60°,90°,120° ,150°arm abduction had not statistical difference (P>0.05).The rot ator cuff(mainly supraspinatus tendon)just went through between acromion and humerous at60°~120°arm positions while not at0°~30°,150°arm positions.Conclusion:The values of A-H at60°~120°arm ab-ducion are smallest and the rotator cuff(mainly supraspinatus tendon)just go through be tween acromion and humerous.So the closest contact between the supraspinatus te ndon and subacromial space occurrs at60°~120°abduction.
