2型糖尿病QTd改变及与心率变异性和室性心律失常关系探讨

【摘要】 目的 :探讨糖尿病QTd、QT间期改变与心率变异性和室性心律失常的关系。方法 :用同步 12导联心电图测定 4 7例糖尿病患者的QTd、QTcmax和经心率校正的QTcmax,同时用Holter监测其心率变异性 (HRV)和室性心律失常发生率 ,并与 37例非糖尿病人进行对比分析。结果 :①糖尿病组QTd、QTcmax较非糖尿病组显著增加 (P<0 .0 5 ) ;②糖尿病组心率变异性指标中SDNN、SDNN -index较非糖尿病组显著降低 (P <0 .0 5 )。糖尿病组QTd与SDANN、SDNN -index及QTcmax与SDNN、SDANN、SDNN -index均存在显著相关性 (P <0 .0 5 ) ;非糖尿病组QTd及QTcmax与心率变异性各指标无明确相关性。③糖尿病组QTd与室性心律失常存在着显著的正相关 ,而非糖尿病组QTd与室性心律失常的发生无相关性。结论 :糖尿病组QTd、QTcmax较非糖尿病组延长 ,且QTd与心率变异性和室性心律失常相关 ,QTd可作为预测糖尿病患者发生恶性室性心律失常、心脏猝死的有用指标。

【Abstract】 Objective: This study aimed to investigate the variation of QT interval dispersion (QTd) and corrected QT interval (QTc) and their correlations with heart rate variability (HRV) and ventricular arrhythmia in type 2 diabetes mellitus (DM2). Methods: QTd, QTmax (maximum QT interval) and QTcmax (maximum QTc) of 47 DM2 patients were measured with 12-lead simultaneous body surface electrocardogram. And their HRV and the onset of ventricular arrhythmia were measured by Holter. The results of these 47 diabetics were compared with the ones of 37 no-diabetics. Results: ①QTd and QTcmax of diabetes group obviously prolonged by comparing to the controls (P<0.05). ②The SDNN and SDNN-index of HRV indexes notably decreased in diabetes group contrasted with the no-diabetics (P<0.05); QTd and SDANN, SDNN-index were notably interrelated as well as QTcmax and SDNN, SDANN, SDNN-index in diabetes group (P< 0.05) ; QTd and QTcmax of no-diabetics showed no significant correlation with HRV indexes. ③QTd showed positive correlation with ventricular arrhythmia in diabetes group but it showed no correlation with ventricular arrhythmia in no-diabetes group. Conclusion: QTd and QTcmax notably prolong in diabetics group by comparing with the ones in no-diabetes group. This study also shows that QTd is well correlated with HRV and ventricular arrhythmia. QTd prolongation is a very useful independent marker of increased ventricular arrhythmia and sudden death of type 2 diabetes mellitus.
