CT导向肺活检标本端粒酶活性检测对肺癌的早期诊断价值

【摘要】 目的 研究肺活检标本端粒酶活性检测对肺癌的早期诊断价值。资料与方法 经X线平片与CT扫描疑诊为早期肺癌 5 2例患者 ,分别进行纤维支气管镜活检与经皮针刺切割活检 ,并使用银染端粒酶重复序列扩增法(TRAP)检测活检标本端粒酶活性 ;而后将早期肺癌 (T1N0 M0 )与非癌病变组对照研究。结果 经手术病理证实为早期肺癌患者 2 2例 ,肺活检标本端粒酶活性表达阳性率为 86 .4 % (19/ 2 2 ) ;经手术或随访 2年以上证实为良性病变者 2 4例 (肺囊肿 3例 ,结核 6例 ,炎性假瘤 5例 ,肺炎 10例 ) ,端粒酶活性表达阳性率为 4 .2 % (1/ 2 4 )。两组有显著性差异 (P <0 .0 1)。结论 端粒酶可以作为肺癌定性的一个敏感的肿瘤标志物 ,结合病理学检查 ,有可能对肺癌作出早期诊断

【Abstract】 Objective To evaluate the telomerase activity of biopsy specimen in diagnosing early lung cancer.Materials and Methods Fiberbronchonscopic and percutaneous cutting needle biopsy were performed in 52 patients with suspected early pulmonary cancer by plain chest film and CT scan. The telomerase activity was measured. The PCR based silver staining telomeric repeat amplification protocol (TRAP) was used for the detection of telomerase activity in 22 patients with early lung cancer (T 1N 0M 0). Control study was conducted with the speciments from 24 patients with benign lesions, including cyst (n=3), tuberculosis (n=6), pseudotumor (n=5) and pneumonia (n=10).Results The positive expression rate of telomerase activity in early lung cancers and benign lesions was 86.45% (19/22) and 4.2% (1/24), respectively, which was significantly higher in early lung cancer than that in benign lesion. The difference between two groups was statistically significant (P<0.01).Conclusion Telomerase activity can be used as a sensitive tumor marker for detecting lung cancer. An early diagnosis of lung cancer can be made when combining telomerase activity with the pathological results.
