【摘要】 目的应用螺旋CT协助确定Krukenberg瘤合并腹膜转移的手术可切除性 ,以帮助制订合理的治疗方案。方法回顾性分析 2 2例手术病理或临床追踪证实为胃肠来源的Krukenberg瘤合并腹膜转移的CT资料 ,各种转移灶的不同CT表现对治疗方法、手术难度与生存时间的影响进行分析 ,以确定螺旋CT在这类患者选择治疗方案 (即是否考虑手术切除 )时的价值。结果卵巢肿块与周围脏器粘连、累及周围血管及腹膜广泛转移 ,特别是形成腹膜肿块者手术难度均增大 ,影响生存率 ;而淋巴结转移对手术难度影响不大 ,但影响生存率。结论螺旋CT能准确显示Krukenberg瘤大部分原发灶和转移灶并对其可切除性提供术前资料 ,为确定治疗方案提供可靠依据更多还原

【Abstract】 ObjectiveTo evaluate the importance of spiral CT in deciding the resectability of Krukenberg tumors with peritoneal metastasis and to help the design of therapeutic program.MethodsTwenty two pathologically or clinically confirmed cases were retrospectively analysed with spiral CT data. Different CT signs of metastatic lesions that might influence therapeutic approach, operative difficulty and survival rate were also considered.ResultsThe difficulty of operation might be greater, and survival time shorter in case of adhesion with nearby organs, vessels invasion and widespread peritoneal metastasis. Richer blood supply of Krukenberg lesions and lymph node metastasis did not increase the difficulty of operation but affected survival time.ConclusionsSpiral CT can accurately demonstrate ovarian and peritoneal metastatic lesions and most of the primary lesions, and provide information for resectability of metastatic tumors before operation. Thus it helps offer reliable ground for decision of therapeutic program.更多还原
