垂体腺瘤MRI与病理对照研究
MRI Appearance of Pituitary Adenomas with Correlation of Pathological Findings
【摘要】 目的 分析垂体腺瘤的MRI表现与手术、病理关系。材料与方法 对242例垂体腺瘤的MRI特征性表现进行分析,并与手术病理对照。结果 大腺瘤/高度>10mm)161例,MRI平扫T1WI等信号101例,略低信号21例,混合信号39例,T2WI等信号112例,高信号11例,混合信号38例,增强后肿瘤实质部分均有强化,出血、囊变、坏死和钙化区无强化,微腺瘤(高度<10mm)81例,MRI平扫表现为T1WI略低信号53例,等信号28例,T2WI等信号42例,高信号39例,注射Gd-DTPA后立即成像,77例呈相对低信号,4例呈等信号。手术和病理发现,T1WI等或略低信号区为肿瘤实质,T1WI低信号、T2WI高信号区为坏死或囊变区,T1WI、T2WI均呈高信号区为出血,手术和病理发现钙化6例,术前MRI未发现。结论MRI能清晰显示垂体瘤的大小、形态、轮廓和与周围结构的关系,可为制定手术方案提供重要依据。
【Abstract】 Purpose: To assess the relationship between the MRI appearance and the surgical and pathological findings of pituitary adenoma. Materials and Methods the characteristic MRI appearances of 242 cases of pituitary adenomas were analyzed and compared with the pathological findings. Results 161 of 242 cases were macroadenomas (> 10 mm in diameter) On unenhanced MR T1- weighted imaging, 101 cases of macroadenomas were isointense, 21 cases were slightly hypo - intensity and 39 cases were mixed signal intensity while on T2 - weighted inages 112 cases were isointense, 11 cases were hyperintense and 38 cases were mixed intensity Following Venous ad- ministration of Gd - DTPA, the parenchyma of macroadenomas showed enhancement; on the contrary, no enhancement could be revealed in the areas of hemorrhage, necrosis and calcification. 81 of 242 cases were microadenomas(<10 mm in diameter). At unenhanced MR im- ages. 53 cases showed hypointense, 28 cases showed isointense on T1WI, while 42 cases showed isointense, 39 cases showed hyperintense on T2WI. On Gd - DTPA T1 - weighted images, 77 cases were hypointense compared to the mormal gland, 4 cases were isointense. Surgical and pathological findings showed that isointense or lower intensity represented parenchyma of macroadenoma. The areas with hypointense on T1WI and hyperintense on T2WI represeuted areas of necrosis or capsulation. The areas with hyperintense both on T1WI and T2WI were hemor- rhage 6 cases of calcification invisible in MRI were revealed by operation and pathology. Conclusion MRI plays very important role in re- vealing size, shape, contour, extension of the tumors and their relations with surrounding tissues, thus offering impotant proof for clinical treatment choices.
