【摘要】 目的 :评价白内障超声乳化并人工晶体植入术 (PHACO +IOL)、小切口手法碎核白内障囊外摘除并人工晶体植入术 (SIMN ECCE +IOL)和白内障囊外摘除并人工晶体植入术 (ECCE +IOL)的疗效。方法 :采用回顾分析对比济南市中心医院白内障中心与 15家基层医院协同完成的 82 2 5例 (10 2 2 2只眼 )白内障手术术后视力、散光及并发症。结果 :术后 3个月视力 0 5或以上视力 3组分别为 96 9% (PHACO)、95 7% (SIMN ECCE)、94 4 % (ECCE) (P >0 0 5 )。术后散光检查 ,PHACO +IOL与SIMN ECCE +IOL分别是 :1w为 (2 32± 0 0 76)D和 (2 4 6± 0 0 68)D ,1个月为 (1 4 5± 0 0 66)D和 (1 4 8± 0 0 69)D ,3个月为(0 97± 0 0 60 )D和 (1 0 5± 0 0 72 )D ,均无显著性差异。ECCE +IOL术后散光 1w为 (4 18± 0 14 1)D ,1个月为 (3 0 0± 0 14 4 )D ,3个月为 (1 83± 0 111)D ,与上述两术式同期相比有显著性差异 ,3种术式的术中术后并发症发生率无显著性差异。结论 :白内障术后视力效果及并发症的发生率不取决于手术方式而取决于手术质量 ,3种术式都可以使病人获得良好视力 ,但PHACO +IOL术式及SIMN ECCE +IOL术式与ECCE+IOL术式相比 ,具有切口小、损伤小、散光轻、恢复快、裸眼视力恢复迅速等优点更多还原

【Abstract】 Objective:To compare and evaluate the theraputic effects of three kinds of cataract surgery,i.e.,①Phacoemulsification with IOL implantation(PHACO);②Small incision manual nucleo fragmentation with IOL implantation (SIMN ECCE);③Extracapsular cataract extraction with IOL implantation(ECCE).Methods:The visual acuity,astigmatism and complications postoperatively in 8225(10?222 eyes)cataract surgery co performed by the cataract center of Jinan Municipal Central Hospital and 15 rural hospitals were analysed and compared.Results:The visual acuity at postoperative 3 month was 0.5 or better in 96.9% of the eyes with PHACO,in 95.8% of eyes with SIMN ECCE and in 94.45% of the eyes with ECCE respectively and there was no statistical difference among them.Postoperative mean astigmatisms(D) in PHACO+IOL group and SIMN ECCE group were:(2.32±0.076) D and (2.46±0.068)D in first week,(1.45±0.066)D and ( 1.48 ± 0.069 )D in first month,(0.97±0.060)D,and (1.05±0.072)D in third month.There was no statistical difference between the 2 groups at the same time.Postoperative mean astigmatisms(D) in ECCE+IOL group were:(4.18±0.141)D in first week,(3.00±0.144)D in first month and (1.83±0.111)D in third month.There were statistical difference between this group with above 2 groups.The percentages of intra and postoperative complications patient were rather low and no statistical difference among the 3 groups.Conclusion:The final recovery of visual acuity and the occurrence of complications after cataract surgery do not depend on the surgical methods but on the surgical quality.The 3 kinds of cataract surgery can bring good visual acuity to the patients but both the PHACO surgery and the SIMN ECCE surgery have some advantages,such as smaller incision,less damage,less astigmatism and rapid rehabilitation of visual acuity.更多还原
