【摘要】 目的 :针对前路“Z”形钛钢板内固定治疗胸腰椎骨折手术创伤大 ,失血多 ,技术要求高等问题进行技术改进。方法 :采用经第 11～ 12肋胸膜外 -腹膜后入路显露 ,对手术操作程序进行了改进。结果 :2 9例胸腰椎骨折并截瘫患者手术均获成功。技术改进后术中平均失血量 475 m l,手术操作时间平均 12 9分钟 ,手术切口平均长度 18cm。随访 3～ 13个月 ,平均 8.6个月。术后无截瘫加重 ,脊柱序列正常 ,无植骨块塌陷及高度丢失现象 ,无假关节形成和内固定失败。神经功能获一级以上恢复者达 93%。结论 :该技术改进具有切口短、失血少、手术创伤较轻、降低手术操作难度的优点。更多还原

【Abstract】 Objective:To describe the results and advantages of the improved procedure for the treatment of thoraco-lumbar fractures with Z-PLATE TM ATL.Methods:The extrapleural and retroperitoneal approach with resection of the eleven-twelfth rib were used, thus improved the procedure for the treatment of thoraco-lumbar fractures.Results:All of the 29 cases had successful surgery ; the average amount of blood loss was 475 ml, the average operation time was 129 minutes and the length of incision was 18cm. After an average of 8.6 months followed-up, none of the patients had neurological deterioration; the spine recovered normal curvature and the fusion rate was 100%. No failure of implant happened. Neurological function was improved at least one grade in 93%. Conclusion:The technical improvements have the advantage of less injury, less blood loss and a short incision.更多还原
