【摘要】 目的　比较有或无可拆除巩膜瓣松解缝线小梁切除术治疗青光眼的疗效和并发症。方法　采用随机对照的临床验证方法 ,将 12 9例 (12 9眼 )青光眼分为试验组 (A组 )和对照组 (B组 )。对 A组施行有可拆除巩膜瓣松解缝线的小梁切除术 ,对 B组施行无可拆除巩膜瓣松解缝线的小梁切除术。A、B两组年龄、性别、青光眼类型、术前视力和眼压、前房深度、%值、视野及房角闭合程度、术前使用降压药种数等临床参数无显著差异。比较两组术后前房形成情况、眼压、滤过泡情况及手术并发症。结果　试验组术后第 1天无一例发生浅前房 ,对照组术后第 1天 12例出现浅前房 ,经统计学处理两组有显著性差异。试验组及对照组术后眼压在术后 3周内对比有显著差异 ,3周后差异无显著性。试验组术后功能性滤过泡累积存活率 86 .6 1± 9.5 1% ,对照组为 71.91± 9.88% ,两组间差异有显著性。试验组术后并发症发生率明显低于对照组。结论　显微小梁切除术联合可拆除巩膜瓣松解缝线术不仅能通过调控缝线来控制滤过量 ,而且能明显减少并发症 ,提高抗青光眼手术成功率。更多还原

【Abstract】 Objective To compare the effectiveness and complication of trabeculectomy with or without suture lysis for scleral flap.Methods In a randomized control clinical trial,129 patients(129 eyes) with glaucoma were divided into study group (group A,64 eyes) and control group (group B,65 eyes).Among them,group A underwent trabeculectomy with suture lysis for scleral flap,while group B underwent trabeculectomy without suture lysis for scleral flap.There were no significant statistical differences between two groups with respect to ages,sex,classification of glaucoma,vision,preoperative Iop,depth of anterior chamber,C/D,comfiguration of chamber angle.After trabeculectomy,depth of anterior chamber,postoperative Iop,filtration blebs and complications were followed up and compared.Results On the first postoperative day,no shallow anterior chamber developed in the study group,but there were 12 eyes with shallow anterior chamber in the control group.Statistically,There was significant difference between two groups.There was significant statistical difference between two groups with respect to Iop in three weeks.The statistical difference of Iop was no significant after three weeks.The success rates of functional filtering blebs in two groups were 86.61±9.51% and 71.91±9.88% respectively,the difference being statistically significant.Complications of group A were much less than those of group B.Conclusion Trabeculectomy with suture lysis for scleral flap not only can regulate the function of filtration and control the Iop,but also decrease the occurrence of complications.更多还原
