【摘要】 报告增殖性糖尿病视网膜病变 (proliferative diabetic retinopathy PDR)、严重非增殖性糖尿病视网膜病变 (severenonproliferative diabetic retinopathy severe NPDR)、糖尿病性黄斑水肿患者激光治疗后视野的改变。方法 :5 2眼分 A、 B、 C三组。A组为无黄斑水肿的 PDR及严重 NPDR,行全视网膜光凝术 (panretinal photocoagulation PRP)。B组为 PDR合并黄斑水肿者 ,行PRP联合黄斑区光凝。 C组为单纯黄斑水肿者 ,行黄斑区光凝。结果 :A组光凝后 ,30°内视野平均光阈值敏感度下降 (P<0 .0 1) ,周边视野暗点增多或增大。B组光凝后 ,30°视野平均光阈值敏感度下降 (P<0 .0 1) ,周边视野暗点增大增多 ,10°内光阈值敏感度下降 (P<0 .0 5 )。 C组 10°内视野平均光阈值敏感度下降 (P<0 .0 5 )。三组激光治疗前后视力无显著区别 ,新生血管明显消退 ,黄斑水肿消退。结论 :全视网膜光凝可有效阻止 PDR的进一步发展 ,防止病人视力进一步下降 ,但降低了视网膜光敏感度 ,且周边部视野暗点增多。黄斑区光凝不损伤黄斑中心视功能 ,对糖尿病黄斑部病变局部代谢的改善 ,促进组织修复有一定临床意义更多还原

【Abstract】 Objective:Report the automated perimetic changes following retinal photocoagulation in proliferative diabetic retinopathy(PDR),severe nonproliferative diabetic retinopathy(severe NPDR)and diabetic macular edema.Methods:52 eyes divided into 3 groups A,B and C.A group 36 eyes with PDR and severe NPDR except diabetic macular edema,underwent Panretinal photocoagulation(PRP).B group 9 eyes with PDR and macular edema,underwent PRP and coagulation in macular region.C group 7 eyes with macular edema,underwent coagulation in macular region.Results:After coagulation,the average retinal sensitivity in the 30° had decreased(P<0 01),peripheral retinal scotoma had increased or become larger in A group;the average retinal sensitivity in the 30° had decreased(P<0 01),peripheral retinal scotoma had increased or become larger,the average retinal sensitivity in the 10° had decreased(P<0 05)in B group;the average retinal sensitivity in the 10° had decreased(P<0 05)in C group.After the treatment,the three groups come out that visual acuity had not significant difference but neovascular had lessened significantly and macular edema had dismissed.Conclusion:PRP is effective in reducing the risk of PDR and visual loss,but decrease the retinal sensitivity,peripheral retinal scotoma increase and beconme large.The coagulation in macular region does not injure the centric visual,and it has some clinic meanings for local metabolize improvement of diabetic retinopathy and promoting tissue repair.更多还原
