【摘要】 目的　探讨血清孕酮测定在诊断难免流产及异位妊娠中的价值。方法　 88例因早孕先兆流产入院的病人 ,每周测定 1次血清孕酮 ,直至流产或安胎成功出院。测定 2 0例异位妊娠病人血清孕酮。测定 5 2例正常宫内早孕妇女血清孕酮作为对照。结果　正常早孕各孕周间孕酮值无显著性差异 (P >0 0 5 )。正常早孕、先兆流产与难免流产间孕酮值有显著差异 (P <0 0 5 ,P <0 0 0 0 5 ) ,正常早孕、先兆流产、难免流产与异位妊娠间均有极显著差异 (P <0 0 5 )。以孕酮 47 0 3nmol/L为临界值 ,鉴别难免流产与早孕、先兆流产的敏感性、特异性及准确性分别为 76 0 %、91 8%、86 5 % ;以孕酮 5 5 6 5nmol/L为临界值 ,在疑似异位妊娠病例中鉴别宫内妊娠先兆流产的敏感性、特异性、准确性分别为85 0 %、89 8%、85 1%。结论　血清孕酮测定在诊断难免流产及异位妊娠中有一定意义。孕酮 <47 0 3nmol/L可作为安胎失败的指标 ;而对疑似异位妊娠病例 ,若孕酮 <5 5 6 5nmol/L ,诊刮的安全系数较高。更多还原

【Abstract】 Objective To assess the value of serum progesterone assay in the diagnosis of inevitable abortion and ectopic pregnancy.Methods Serum progesterone of 88 patients admitted to the hospital for threatened abortion of early pregnancy were assayed once a week until abortion or successful treatment for threatened abortion.Also 20 serums of ectopic pregnancy were assayed .The comparison group was 52 normal intrauterine early pregnancys.Results There was no significant difference of progesterone levels among every gestational weeks in the normal early pregnancy group(P>0 05).The progesterone level of inevitable abortion was significantly lower than that of normal pregnancy and the ratened abortion(P<0 05,P<0 0005).And there was also significant difference between normal pregnancy,threatened abortion,inevitable abortion and ectopic pregnancy (P<0 05).The sensitivity,specificity and accuracy were 76%,91 8%,86 5%,respectively,with 47 03nmol/L as the critical value in screening inevitable abortion.And if the critical value was 55 65nmol/L,the three rates would be 85%,89 8% and 85 1% respectively in identification of intrauterine threatened abortion among patients suspected to be ectopic pregnancy . Conclusion The serum progesterone assay is valuable in detecting inevitable abortion and ectopic pregnancy.Serum progesterone <47 03nmol/L may be an indicator of failed treatment for threatened abortion;and diagnostic curettage can be performed without doubt in patients suspected of ectopic pregnancy with serum progesterone<55 65nmol/L.更多还原
