【摘要】 目的 :本文分析了左室射血期心尖部的受力状态 ,并以此为据 ,首次系统观察了心尖部的室壁运动。方法 :适当降低超声仪前场增益 ,局部放大心尖部图像 ,动态观察 :在冠心病患者 ,在胸闷、胸痛间断发作大于一年又无法确诊为冠心病者 ,大都能看到心尖部室壁运动的异常现象。而大部分正常人的心尖部室壁运动正常。结果 :异常现象是指 :在收缩期心室壁作向心运动时 ,心尖部局部室壁作离心性运动 ,舒张期它们各自的运动方向相反。其异常运动幅度有轻、中、重之分。结论 :心尖部室壁运动的异常现象是缺血缺氧的结果 ,是长期胸闷、胸痛症状最容易捕捉到的形态学表现 ,是心尖部室壁瘤逐渐形成的基础 ,是心肌早期缺血的观察窗口更多还原

【Abstract】 Objective:To analyze the moving of left ventricular apical wall during the ejection extensively and systematically with 2D echocardiographyMethods:By reducing the proximal field gain,enlarging the apical areas,observing in dynamics.Results:A kind of abnormal apical wall has been found in some CAD patients or some with obviously precardial pain and dullness for many years in normal personsThe apical local wall moved outward during the ejection,while other ventricular wall performed concentric contractionAnd during the diastolic,it would move inside while others moved outwardThe apical abnormal motion intensity was mild,moderate or severe.Conclusion:The apical wall motions may be an unique morphological change in CAD person with long term precardial pain and dullnessPerhaps it is the basis for apex ventricular aneuryam formation and a window through which we can monitor the development of myocardium ischemia early更多还原
