【摘要】 目的　研究降纤酶治疗急性脑梗死的机理及用药剂量　方法　对照组应用低分子右旋糖酐50 0 ml加复方丹参 2 0 ml静滴 ,每天 1次 ,共 1 4天 ;观察组基础治疗同对照组 ,另在发病 6～ 72 h内加用降纤酶 1 0 u加生理盐水 1 50 ml静滴 ,30～ 60 min滴完 ,每天 1次 ,以后隔日静滴降纤酶 1 0 u,共 3次 ,总量达 30 u,5天为 1疗程 ,不用其他抗凝溶栓、抑制血小板聚集药物。于治疗前及治疗后第 1、 3、 7、 1 4、2 8天采用 ESS各评定 1次 ,分别计算两组病人 ESS的平均减少分数　结果　在起病 2 4 h内开始用药者 ,两组在各个时点上 ESS的平均减少分数比较 ,观察组疗效优于对照组 ( P <0 .0 0 1 ) ;而在起病 2 4～ 72 h开始用药者 ,两组间 ESS的平均减少分比较 ,无显著性差异 ( P >0 .0 5)　结论　降纤酶是急性脑梗死超早期静脉溶栓治疗中可以广泛应用的较好药物更多还原

【Abstract】 Objective To study the mechanism and dosage of defibrinogenase to treat acute brain infarction Method control group,Dextran 10 500ml with salvia miltiorrhiza compound injection 20ml,ivgtt qt for 14 days;treating group,the basic treatment is the same as that of the control group,within 6~72 hours after onset of acute brain infarcton,defibrinogenase lou with nomal saline 150ml,ivgtt 30~60 minutes the drip was completed after that every other day defibrinogenase lou ivgtt all for three times,the total amout reaches to 30u, 5 days as one course of treatment,without using anticoagulants and antiplates before treatment and after treatment’s 1,3,7,14,28 days,each day give an assessment by ESS method Results Therapy beginning within 24 hours after onset,the treating group’s curative effect was better than that of the conrol group(P<0 001),but therapy beginning within 24~72 hours after onset,the two groups’ curative effect had no significant change(P>0 05) Conclusion Defibrinogenase is one of the ideal drug can be widely used for curing early brain infarction更多还原
