【摘要】 目的 :观察宫血停合剂 (由丹参、赤芍、桃仁、三棱、莪术、蒲黄、益母草、五爪龙、枳壳组成 ,以下简称宫血停 )治疗血瘀型崩漏的止血疗效及探讨其止血机理。方法 :通过观察 37例血瘀型崩漏病人治疗前后的出血时间、出血量的改变来判断宫血停的止血疗效 ;通过对 8例病人治疗前后血分析、血液流变学、凝血三项、前列腺素等指标的测量 ,推断其止血机理。结果 :宫血停对血瘀型崩漏病人在经期的缩短和维持正常经量方面有较显著的疗效 (止血后的调理周期与原周期比 P <0 .0 1)。治疗后红细胞平均血红蛋白浓度 (MCHC)、 6-酮 -前列腺素1α ( 6-k -PGF1α)、RBC压积 (HCT)、纤维蛋白原 (FIB)比原来降低(P <0 .0 5) ;全血粘度、红细胞体积分布幅度—变异系数 (RDW -CV)、血小板体积分布幅度 (PDW )、平均血小板容积 (MPV)比原来升高 (P <0 .0 5) ;其余指标均无显著性改变 (P >0 .0 5)。结论 :宫血停对血瘀型崩漏有一定疗效 ,总有效率为 91.89%。其止血机理可能在于 :①提高血小板的聚集粘附能力 ;②促纤溶活性 ;③调节前列腺环素的释放更多还原

【Abstract】 A prospective clinical trial was carried out to explore the hemostatic effect of Gongxueting Mixture on metrorrhagia and metrostaxis with blood stasis syndrome and its mechanism. Bleeding time and bleeding volume in 37 cases were detected before and after treatment to assay the hemostatic effect. Indexes of blood analysis, blood rheology, blood coagulation and prostaglandin in 8 cases were determined to study the mechanism. The results showed that Gongxueting Mixture shortened the menstrual period and regulated the volume of menses (P< 0.01 ). After treatment, mean corpuscular hemoglobin concentration, 6-keto-PGF 1α level, hematocrit and fibrinogen content were decreased (P< 0.05 ), and the whole blood viscosity, red cell volume distribution amplitude, platelet volume distribution amplitude and mean platelet volume were increased (P< 0.05 ). No significant changes were found in the other indexes (P> 0.05 ). It is concluded that Gongxueting Mixture is effective for metrorrhagia and metrostaxis with blood stasis syndrome (the total effective rate being 91.89% ), and enhancement of platelet aggregation, promotion of fibrinolytic activity and regulation of prostaglandin release may be its hemostatic mechanism.更多还原
