运动单位数目测定:正常值的建立及其在下运动神经元病变中的价值
Motor unit number estimation:establishment of normal data and evaluation on lower motor neuron affection
【摘要】 目的 :建立运动单位数目测定 (MUNE)的正常值并初步探讨其在下运动神经元病的临床应用价值。方法 :应用VikingⅣ型肌电图仪中的“Poisson分布法”程序 ,对 75例健康人和 5 6例下运动神经元病变者选择趾短伸肌和鱼际肌进行测定。记录和刺激电极均为表面电极。结果 :在对照组 ,其结果显示可重复性好 ;<6 0岁时 ,运动单位 (MU)数与年龄、性别和侧别无关 ;>6 0岁时 ,MU数显著减少。在病人组 ,结果的可重复性更佳 ;运动神经元或轴突受损者均显示为运动单位 (MU)数减少 ;随着病情的好转 ,功能性MU数增加。结论 :MUNE可定量地评价运动单位的丧失 ,方法简便、快速、无创 ,是检测下运动神经元性病变的理想手段。
【Abstract】 Objectives:To establish the mormal data base and investigate the clinical significance of motor unit number estimation(MUNE).Methods:A statistical method program in Nicolet Viking Ⅳ was applied to estimate the number of motor unit (MU) of extensor digitorum brevis (EDB) and thenar muscles in 75 healthy volunteers and 56 patients with varied lower motor neuron affection, in which surface electrodes were used for both stimulating and recording.Results:In controls, the findings by the MUNE technique were of good reproducibility,and there was no correlation of the MU number with either age, sex or side for those with the age under 60 years,whereas the functioning MU number reduced significantly for those over 60 years. In patient group, the results were better reproducible than those in the controls,and the MU number markedly reduced,and the increase of MU number was significantly associated with clinical recovery.Conclusion:The technique of MUNE might be served to evaluate quantitatively the motor unit loss and be an ideal means for testing the lower motor neuron affection, for being simple, fast and non invasive.
