血清肌钙蛋白Ⅰ在系统性红斑狼疮心脏损伤的意义

【摘要】 目的:探讨血清肌钙蛋白Ⅰ(troponin Ⅰ,Tn-Ⅰ)在诊断系统性红斑狼疮(SLE)心脏损害中的意义。方法:本研究以酶联免疫吸附法(ELISA)检测45例SLE患者血清Tn-Ⅰ浓度,并根据病情分组与正常对照组比较,其阳性率同UCG、ECG、CK-MB的阳性率进行比较。结果:45例SLE根据病情分三组,活动组10例;可能活动组15例,病情稳定组20例,其血清浓度分别为0.62±0.26ug/L、0.47±0.30ug/L、0.14±0.08ug/L,显著高于正常对照组0.01±0.04ug/L(P<0.05),同一检验方法活动组阳性率高于可能活动组和病情稳定组(P<0.05),并同病情活动组呈正相关,组间阳性率的比较,Tn-Ⅰ阳性率显著高于UCG、ECG、CK—MB(P<0.05)。结论:血清肌钙蛋白Tn-Ⅰ的检测可以作为心肌受累及受累程度敏感性的判断指标。

【Abstract】 obuective To study significance of serum troponin I(Tn-1) in cardiac involvement of patients with systemic lupus erythematosus.Method: Serum Tn-1 concentrations were measured in 45 patients with SLE by the enzyme-linked immunosorbent assay (ELISA).All patients were divided into three groups depending on disease activity and severity.The results of Tn-1 of the patient groups were compared with normal control,and the positive rates of serum Tn-1 were compared with those of UCG,ECG,CK-MB.Result: The levels of serum Tn-1 of the active groups(10 cases),the possible active groups( 15 cases) and inactive groups (20 cases) were 0.62±0.26 ug/L,0.47±0.30ug/L,0.14±0,08ug/L respectively.They were higher than normal control(0.10±0.07ug/L) (P<0.05).The positive rates of Tn-I of the active group was higher than those of other groups (P<0.05),and there was a positive correlation with disease activity.In comparing with other tests,it was found that positive rates of Tn-1 was higher than those of UCG.ECG.CK-MB(P<0.05).Conclusion: The measurement of serum Tn-I-can be used as an index for assessing cardiac involvement and exploring the seveity in SLE patients.
