冠心病血瘀证单核细胞凝血与纤溶活性研究

【摘要】 采用发色底物法检测冠心病血瘀证和非血瘀证患者外周血单核细胞 (PBMC) 促凝活性(PCA) ,组织型纤溶酶原激活物 (t- PA) 及其抑制物 (PAI- 1) 活性 ,研究冠心病血瘀证患者外周血单核细胞凝血 /纤溶活性改变及其在发病中的意义。 80例冠心病患者作为病例组 ,其中血瘀证组 3 0例 ,非血瘀证组 50例 (包括痰浊证 2 7例 ,气滞证 2 3例 ) ,健康对照组 2 0例。结果表明 ,冠心病血瘀证患者 PBMC PCA显著高于非血瘀证组和正常对照组 (P<0 .0 1) ;但血瘀证和非血瘀证患者 PBMC的 PAI-1活性无显著差异 (P>0 .0 5 ) ,均高于正常对照组 (P<0 .0 1) ;血瘀证患者 PBMC的 t-PA活性显著低于非血瘀证组 (P<0 .0 1) 。故得出冠心病血瘀证患者单核细胞促凝活性增高 ,纤溶活性下降 ,上述凝血 /纤溶改变可能是血瘀证形成的重要因素之一 ,提示冠心病血瘀证患者单核细胞已处于血栓前状态

【Abstract】 Objective:To explore the essential changes of Coronary Heart Disease patients with blood stasis(CHDS) of Traditional Chinese Medicine(TCM),We examined the expression of human peripheral blood monocyte(PBMC)pro coagulant activity(PCA)and fibrinolytic components such as tissue type plasminogen activator(t PA) and its inhibitor(PAI 1)of CHDS patients.Methods:All the parameters were measured by chromogenic substrate method respectively among 80 patients of coronary heart disease(30 with blood stasis,27 with phlegm damp and 23 with Qi stagnation).20 healthy persons were recruited as normal control group.Results:Procoagulant activity(PCA) expressed by perpherial blood monocyte for CHDS patients was much higher than that for NCHDS patients and control group( P <0.01).PAI 1 activity expressed by PBMC for CHDS was much higher than that for control group( P <0.01).No significant difference of PAI 1 activity of PBMC was found between CHDS group and NCHDS groups( P >0.05).t PA activity expressed by PBMC for CHDS patients was declined compared with NCHDS patients and control group( P <0.01).Conclusions: It was showed that CHDS patients’ PCA expression was greatly increased and their fibrinolytic activity expression was declined.These changes indicate that reduced fibrinolysis function and increased coagulant activity were the essential pathogenic changes for the development of CHDS,and these patients were in the prethrombotic state.
