【摘要】 目的　研究、评价顺铂 -白蛋白微球 (CDDP- AMS)经舌动脉灌注治疗舌癌的临床效果。初步探讨顺铂 -白蛋白微球在头颈部肿瘤治疗上的价值。方法　利用平均 5 6 .3μm顺铂 -白蛋白微球 10 0 m g(含 CDDP13.6 mg)经舌动脉灌注 ,治疗晚期舌癌患者 18例 ,观察分析治疗效果。结果　顺铂 -白蛋白微球 CDDP- AMS经舌动脉灌注治疗舌癌 18例 ,近期疗效达特效者 10 0 % ,配合其它治疗后 ,经 5年观察 ,原发灶无复发。结论　顺铂 -白蛋白微球舌动脉灌注治疗舌癌具有确切的治疗效果 ,可以做为舌癌的综合治疗措施之一更多还原

【Abstract】 Objective To study and evaluate the clinical effect of cisplatin loaded albumin microspheres(CDDP AMS) infusion through lingual artery in treating carcinoma of tongue, to conduct an initial discussion on the value of cisplatin loaded albumin microspheres in treating tumors in head and neck.Material and methods Biodegradable and nonantigenic in vivo albumin was selected as a carrier, and encapsulated the anticancer agent cisplatin to treat 18 patients with terminal carcinoma of tongue by infusion through lingual artery with cisplatin loaded albumin microspheres 56.3μm in diametre, to observe and analyze the curative effect.Result In the treatment of 18 patients with carcinoma of tongue by CDDP AMS infusion through tongue artery, 4 patients in this group are performed with focal excision 4 to 6 weeks after CDDP AMS infusion, no carcinoma cells are found in the continuous pathological slices of the specimen, the lingual ulcer of the other 14 patients has healed completely 6 weeks after embolism, the cure rate reaches 100%. It is found in the 18 patients that P of all the check up index of body fluid immunity is lower than 0.05 and there are prominent differences after the pairedt examination. Matched with other therapy, no recurrence is found on the original focus after five years of observation.Conclusion The results indicated: infusion of cisplatin loaded albumin microspheres can block the blood supply to the tumor, which caused local ischemia and hypoxia. As the microspheres degradation, the drug was released to the tumor, and caused cytotoxic effect to kill the carcinoma cells. It may also provoked an immunoreaction to be engaged in copying with the tumor. CDDP AMS superselective embolism treating carcinoma of tongue was a simple method of high localization, effectiveness lasting and with minimal side effects. It can be used as one of the malignant tumor comprehensive treatment methods, which shows wide prospect for the treatment of oral and maxillofacial tumor.更多还原
