聚合酶链反应检测卡氏肺胞子虫
Detection of Pnumocystis Carinii with Polymerase Chain Reaction
【摘要】 随着艾滋病的出现和流行 ,卡氏肺孢子虫肺炎 ( PCP)的发病率急剧增高 ,PCP的症状重 ,缺乏相应的体征 ,病死率高达 2 0~ 6 0 % ,早期诊断、及时治疗是降低死亡率的关键。 Wakefield AE等首次将核酸扩增技术 ( PCR)应用于卡氏肺孢子虫的检测 ,随后陆续有针对不同 Pc靶基因 PCR的研究报道。本文采用 PCR技术检测醋酸可的松诱导的 SD大鼠 PCP模型 ,结果从 2 5只 PCP大鼠的支气管肺泡灌洗液中检出 2 0例阳性 ,阳性率为 80 % ( 2 0 / 2 5 ) ,较传统吉姆萨染色方法阳性率 44 .0 % ( 11/ 2 5 )有明显的提高 ,而 7只对照组大鼠两种方法检测均为阴性。随着标本处理的进一步简化 ,PCR将有望成为临床 PCP的病原学诊断手段之一
【Abstract】 With the emerging and development of AIDS epidemic,the incidence of Pnumocystis carinii Pneumonia(PCP)has increased dramatically and its mortality could reach as high as 2060%.Early diagnosis and timely medical intervention are the key elements in reducing its death rate.Wakefield AE et al initially introduced DNA amplification in detection of Pnumocystis carinii(Pc),and several other PCRs targeting at different genes were reported afterward.In our study,PCR was used in detection of Pneumocystis carinii in bronchial aloveolar lavage fluids(BALFs)from rat models with PCP induced by biweekly injection of acetic cortisone.In 25 rats with PCP,20 BALFs were positive by PCR compared to only 11 positive by traditional Giemsa staining,and 7 BALFs from negative control were all negative either by PCR or Giemsa staining.The result shows that PCR is more sensitive than traditional staining in detection of Pc,and is a promising approach of etiological diagnosis in clinical practice as sample processing was simplified.
