【摘要】 目的 :观察小剂量甲状腺素治疗慢性严重充血性心力衰竭的疗效与可行性。方法 :30例老年慢性严重充血性心力衰竭病人入院时查血 FT3 、FT4 、TSH、HR和心功能。除按常规治疗外 ,每日加服甲状腺素片2 0～ 40 mg。 1月后复查血 FT3 、FT4 、TSH、HR及心功能。设中年无心力衰竭对照组 2 0例。结果 :1 30例老年慢性严重充血性心力衰竭病人治疗 1月后 2 9例疗效佳 ,只有 1例心功能无明显改善。未诱发心绞痛和心房纤颤等并发症。 2老年心力衰竭组与中年对照组比较 ,FT3 和 FT4 显著降低 ( P<0 .0 0 0 5) ;TSH差异无显著性 ;HR差异有显著性 ( P<0 .0 0 0 5)。 3甲状腺素治疗组治疗前后自身比较 ,治疗 1月后 FT3 和 FT4 明显升高 ( P<0 .0 0 1 ;P<0 .0 0 5) ;TSH和 HR变化无显著性差异。 4甲状腺素治疗组治疗后与对照组比较 ,FT3 、FT4 、TSH和 HR差异均无显著性 ( P>0 .1 0 ;P>0 .2 5;P>0 .2 5;P>0 .2 5)。结论 :小剂量补充甲状腺素能提高血中游离甲状腺素含量 ,对纠正老年慢性严重充血性心力衰竭有效 ,未发现明显不良反应。更多还原

【Abstract】 Objective:To study the effecfs and feasibility about thyroid hormone in the old men with chronic severe congestive heart failure.Methods:To examine the serum FT 3,FT 4,TSH,HR,and heart function of the heart failure patients when hospitalization.To examine them again one month later treated by thyroid homone 20 40 mg everyday.To exaimine FT 3,FT 4,TSH and HR of the control group.Results:①Twenty nine patients got progress of the heart function and only one was still in grade Ⅳ one month later.No angina or atrial fibrilation attack.②FT 3 and FT 4 were significantly lower in the old men with chronic severe congestive heart failure than in the middle age men with no heart failure ( P <0 0005; P <0 0005).The difference of TSH was no significant.The difference of HR was significant ( P <0 0005).③FT 3 and FT 4 were significantly higher after treatment than before treatment ( P <0 001; P <0 005).The differences of TSH and HR were no significant.④The differences of FT 3,FT 4 TSH and HR were all no significant both in heart failure group treated by thyroid hormone and the control group.Conclusion:This study indicates that small doses of thyroid hormore were beneficial in the old men with chronic severe congestive heart failure.更多还原
