【摘要】 目的：探讨胎盘成熟度分级与胎肺成熟度之间的联系，以提高对胎儿的监测水平。方法：在Ｂ超下对中山一院１９９４年８月～１９９５年１月，孕龄３０～４２周的单胎妊娠孕妇９１例进行胎盘成熟度分级和胎肺成熟度检查。结果：在进行监测的９１例孕妇中，０级胎盘分布孕龄较Ⅰ～Ⅲ级胎盘组明显小（Ｐ＜００５）。羊水中磷脂酰胆碱或称卵磷脂／鞘磷脂值（Ｌ／Ｓ）与孕周、胎头双顶径（ＢＰＤ）、股骨长度（ＦＬ）呈直线正相关。胎盘成熟度分级与磷脂酰甘油（ＰＧ）阳性率无相关性。在正常妊娠情况下，胎盘成熟度分级、胎肺成熟与孕龄呈正相关。在病理妊娠情况下，两者无相关性。结论：在正常情况下，胎盘成熟度分级可作为衡量胎肺成熟的指标；在病理情况下，胎盘成熟度分级不能反映胎肺成熟情况，不能作为衡量胎肺成熟的指标更多还原

【Abstract】 Objective: To study the relation between placental mature grading and fetal pulmonary maturity to improve the level monitoring fetus. Methods: All study subjects were selected from gravida having antenatal visit in the First Affiliated Hospital from August 1994 to January 1995. The age of pregnant women arranged from 22 to 35 years. They had been pregnant for 30 to 42 weeks with a single fetus. After the placental maturity had been graded according to the Grannum′s criterila under B ultrasoundgraphy. Fetal pulmonary maturity had been done by measuring phosphatidylglycerol(PG) and phosphatidylcholine or lecithin/sphingomyellin ratio (L/S) with Thin Layer Chromatography. Results: Gestation ages of O Grad was much smaller than that of Ⅰ～Ⅲ Grads ( P <0 05) among 91 gravida. The L/S ratio correlated positively with gestational ages、 biparietal diameter(BPD) and femur length(FL). Placental mature grading had negative correlation with phosphytidlyglycomy. Placental mature grading and fetal pulmonary maturity correlated positively with gestation ages in normal pregnancy, but they had no correlation in abnormal pregnancy. Conclusions: Placental mature grading may be a substitute predicting fetal pulmonary maturity in normal pregnancy, but it is not ture in abnormal pregnancy.更多还原
