糖尿病肾病前列腺素变化与肾脏损害的关系

【摘要】 ①目的 探讨糖尿病肾病(DN)病人前列腺素系统(PGs)代谢异常在肾脏损害中的作用机制及临床意义。②方法 选择72例DN肾功能代偿期病人,采用放射免疫测定法分组测定在不同程度肾脏损害时尿血栓素B2 (TXB2 ),6-酮-前列腺素F1α(6-K-PGF1α)排泄量,并观察与尿清蛋白排泄率(UAER)、血肌酐(SCr)、内生肌酐清除率(CCr)等的关系。③结果 DN各组尿TXB2 较对照组均显著升高(F= 5.28,q= 3.46~5.34,P< 0.05,0.01),且随肾脏损害加重呈递增趋势,与UAER呈正相关(r= 0.27,P< 0.05);6-K-PGF1α的升高则以早期正常清蛋白(Alb)尿组最高(F= 15.76,q= 7.93~8.26,P< 0.01),随肾脏损害加重而下降,并与CCr呈正相关(r= 0.30,P<0.05),DN各组CCr与对照组比较差异无显著性,尿TXB2/6-K-PGF1α在肾功能代偿期亦无显著变化。④结论测定尿TXB2,6-K-PGF1α及其比值有助于了解肾脏损害程度及肾功能状态,并可根据DN的不同时期制订相应的治疗措施

【Abstract】 Objective\ To investigate the effect of metabolic abnomality of prostglandin system (PGs) in the kidney damage in the patients with diabetic nephropathy (DN) and its clinical significance.\ Methods\ Urinary thromboxane B 2(TXB 2) and 6 keto prostglandin F 1α (6 K PGF 1α ) of 72 cases with DN were detected by radioimmunoassay and the relations between them and urinary albumin excretion rate (UAER), serum creatinine (SCr), endogenous creatinine clearance rate (CCr) were observed. Results\ Urinary TXB 2 significanly increased in all DN groups ( F=5.28,q=3.46-5.34,P <0.05,0.01). The more seriously the kidney was damaged, the more urinary TXB 2 increased. There was a positive correlation between it and UAER ( r=0.27,P < 0.05 ).The level of urinary 6 K PGF 1α was the highest in early stage group with normal urinary albumin (UA) ( F=15.76,q= 7.93 -8.26,P <0.01). The worse the kidney was damaged, the more it decreased, but it had a positive correlation with Ccr ( r= 0.30 ,P <0.05). The difference of Ccr between the DN groups and control group was not significant. Urinary TXB 2/6 K PGF 1α did not have remarkable change in compensatory stage of renal function.\ Conclusion\ By detecting urinary TXB 2,6 K PGF 1α and their ratio, it is helpful to know how seriously the kidney was damaged and to make corresponding therapeutic measures according to different stages of DN.\;
