移植肺的肺功能保护实验研究

【摘要】 目的:探讨肺表面活性物质对移植肺的肺功能保护作用。方法:40只Wistar成熟大白鼠为研究对象。戊巴比妥钠腹腔内麻醉下经气管切开口行气管插管,施行人工呼吸并经气道注入肺表面活性物质(SF)。经肺动脉灌注乳酸林格氏液(0~4℃),并低温保存肺。结果:冷藏后1小时及2小时,气道内单纯注入SF的S组以及并用气道加压(0.98kPa)SP组的静态肺顺应性(SLC)均未见明显降低。而未注入SF的NP(气道内未加压)及P(气道内加压)组的SLC在冷藏后2小时均明显降低(P<0.05)。冷藏后1小时P组SLC明显高于NP组(P<0.05)。SP及S组的肺洗液中磷脂质与蛋白质比率明显高于NP组及P组(P<0.05)。结论:SF对移植肺的肺功能具有明显保护作用,气道内加压将减慢SLC降低速度。

【Abstract】 Objective: Effects of surfactant replacement were evaluated on graft pulmonary function in rats. Method: Forty adult Wistar rats were anesthetized with an intraperitoneal injection of pentobarbital sodium(30 mg·kg-1), and intubated through tracheolomy. All rats were ventilated with a ventilator. Surfactant of 50 mg was injected into lungs through tracheal. Cold Ringer’s solution(0℃-4℃)was infused into the lungs through pulmonary artery, and then they were kept in cold solution. Result: The static lung compliance (SLC)in the both surfactant-replaced S(no airway pressurc)and SP(0.98kPa airway pressure)groups did not markedly decrease after cold storage. However,SLC in the no surfactant-replaced NP (no airway pressure)and P (0.98kPa airway pressure) groups both significantly decreased after cold storage. SLC was higher after lh in the P group than NP group(P<0.05). The ratios of lipids/proteins (lung lavaged solution)in the S and SP groups were significantly increased compared with those of the NP and P group (P<0.01). Conclusion:Surfactant replacement and airway pressure of 0.98 kPa can protect pulmonary function of transplanted lung from decreasing.
